基于louvain算法的真实世界生脉注射液治疗老年心血管疾病用药方案的复杂网络分析

摘　　要：目的:筛选归纳出生脉注射液治疗老年心血管病病因及并发症的临床联合用药组合方案。方法:提取全国24家三甲医院信息系统(hospital information system,HIS)数据库中使用生脉注射液治疗的6382名老年心血管病患者的用药信息,利用Louvain算法对数据进行复杂网络分析。结果:针对病因治疗可以应用生脉注射液联合降压降糖降脂和扩冠抗栓类药物;合并呼吸道感染的患者,可以应用“生脉注射液+莫西沙星+痰热清注射液+盐酸氨溴索”清热化痰抗感染;合并胃部溃疡的患者,可以应用“生脉注射液+奥美拉唑”抑酸止痛;合并恶心呕吐的患者,可以应用“生脉注射液+甲氧氯普胺+脂肪乳”和胃止呕;合并水肿的患者,可以应用生脉注射液+螺内酯+呋塞米+参附注射液利水消肿。结论:生脉注射液联合其他中成药和西药治疗老年心血管病体现了中西结合治疗的优势,文章同时探讨了中成药在老年心血管病便秘人群中的应用以及老年心血管病人群的用药特点,有利于今后更好的展开老年心血管病个体化综合治疗。Objective:To screen and summarize the clinical combination regimen of Shengmai Injection in treating the causes and complications of cardiovascular disease in the elderly.Methods:The medication information of 6382 elderly cardiovascular disease patients treated with Shengmai Injection was extracted from the hospital information system(HIS)database of 24 tertiary hospitals across the country,and the Louvain algorithm was used to perform complex network analysis on the data.Results:For the treatment of the cause,Shengmai Injection combined with antihypertensive,hypoglycemic and lipid-lowering and coronary dilation antithrombotic drugs can be used;for patients with respiratory tract infection,“Shengmai Injection+Moxifloxacin+Tanreqing Injection+Ambroxol Hydrochloride”clears away heat,resolves phlegm and resists infection;patients with gastric ulcers can use“Shengmai Injection+Omeprazole”to suppress acid and relieve pain;patients with nausea and vomiting can use“Shengmai Injection+A”“Oclopramide+fat emulsion”can be used to relieve vomiting and relieve stomach vomiting;for patients with edema,Shengmai Injection+Spironolactone+Furosemide+Shenfu Injection can be used to dilute water and reduce swelling.Conclusion:Shengmai injection combined with other Chinese patent medicines and Western medicines in the treatment of cardiovascular disease in the elderly reflects the advantages of combined Chinese and Western medicine.The article also explores the application of Chinese patent medicines in the elderly with cardiovascular disease and constipation as well as the medication characteristics of the elderly with cardiovascular disease.,which is conducive to better developing individualized comprehensive treatment of elderly cardiovascular diseases in the future.
