摘　　要：糖尿病肾病的病性为本虚标实,其以脾肾亏虚为本,痰浊、瘀血相互搏结,损伤肾络为患,“痰瘀互结”是病机关键。脾为后天之本,肾为先天之本。脾胃运化失职,肾脏封藏失司,脾肾亏虚影响水谷精微的运化摄纳,使水液潴留,化液成痰,痰阻气血,血气郁结,痰瘀互结。而痰瘀内生,壅滞经络,会加重脾络肾络受损,可见痰瘀互结与脾肾亏虚又互为因果。故以“培补脾肾,通络化痰,活血化瘀”之中医治法论治糖尿病肾病,同时强调“标本兼治,补虚泄实”来使正气恢复,邪气散出,以期在临床上对糖尿病肾病的中医药治疗提供参考。The nature of diabetic nephropathy is deficiency in origin and excess in excess.It is based on spleen and kidney deficiency.Phlegm and blood stasis interact with each other and damage the kidney collaterals.“Phlegm and blood stasis interact with each other”is the key to the pathogenesis.The spleen is the foundation of acquired diseases,and the kidneys are the foundation of congenital diseases.The spleen and stomach fail to transport and transform,and the kidneys lose their ability to store and store.Insufficiency of the spleen and kidney affects the transport,transformation and absorption of water and grain essence,resulting in water retention,fluid transformation into phlegm,phlegm blocking Qi and blood,blood Qi stagnation,and phlegm and blood stasis.However,endogenous phlegm and blood stasis will stagnate the meridians,which will aggravate the damage to the spleen and kidney meridians.It can be seen that the mutual accumulation of phlegm and blood stasis and the deficiency of the spleen and kidneys are cause and effect of each other.Therefore,the traditional Chinese medicine treatment of diabetic nephropathy is based on“tonifying the spleen and kidneys,unblocking collaterals and reducing phlegm,activating blood circulation and removing blood stasis”,and also emphasizes“treating both the symptoms and root causes,tonifying deficiency and releasing excess”to restore healthy Qi and dissipate evil Qi,in order to achieve clinical success.Provide reference for traditional Chinese medicine treatment of diabetic nephropathy.
