选择性颈肌移除及周围神经切断术治疗脑瘫合并局限性颈肌痉挛效果分析

摘　　要：目的探讨选择性颈肌移除及周围神经切断术治疗脑瘫合并局限性颈肌痉挛的方法和效果。方法回顾性分析郑州大学脑瘫外科研究治疗中心2005-09—2010-09收治的60例脑瘫合并局限性颈肌痉挛患者的临床资料。依据头颈部异常扭动的类型,经过运动解剖学分析,明确各型中参与颈部异常运动的原动肌和协同肌,并通过解剖观察20具(40侧)颈部标本,结合原动肌的形态、位置、神经支配,实施选择性颈肌移除及周围神经切断术。评价手术效果。结果近期有效率达100%,经过10年以上的跟踪随访,治愈率达90%。结论选择性颈肌移除及周围神经切断术是治疗脑瘫合并局限性颈肌痉挛患者安全、有效的术式。Objective To investigate the method and effect of selective cervical muscle removal and peripheral neurotomy in the treatment of cerebral palsy complicated with localized cervical muscle spasm.Methods A retrospective analysis was performed on the clinical data of 60 patients with cerebral palsy complicated with localized cervical muscle spasm admitted to the Cerebral Palsy Surgery Research and Treatment Center of Zhengzhou University from September 2005 to September 2010.According to the type of abnormal twisting of the head and neck,the prime mover and synergistic muscles involved in the abnormal movement of the neck were identified after motor anatomical analysis.Twenty neck specimens(40 sides)were anatomically observed,and the morphology,position and innervation of the prime mover muscle were combined to perform selective neck muscle removal and peripheral neurotomy.Results The recent effectiveness rate is 100%,and the curative rate reaches 90%after more than 10 years of follow-up.Conclusion Selective cervical muscle removal and peripheral neurotomy is a safe and effective procedure for the treatment of patients with cerebral palsy and rotational localized cervical spasm.
