活血止痛方联合腰椎巴布膏治疗腰椎间盘突出症急性期(气滞血瘀证)疗效观察

摘　　要：目的观察活血止痛方联合活血通络腰椎巴布膏治疗腰椎间盘突出症急性期(气滞血瘀证)临床疗效。方法选取80例腰椎间盘突出症急性期(气滞血瘀证)患者,用随机数字表法将患者分为两组各40例。在常规治疗的基础上,对照组予活血通络腰椎巴布膏,观察组予活血止痛方联合活血通络腰椎巴布膏,均连续治疗10d。比较治疗前后视觉模拟量表(VAS)评分、Oswestry功能障碍(ODI)指数、下腰痛日本骨科协会量表(JOA)评分、中医证候积分、临床疗效及不良反应情况。结果治疗后,两组VAS、ODI、中医证候积分(腰部疼痛;下肢麻痛;腰肌僵硬,活动受限;痛有定处,痛处拒按)均较治疗前下降(P<0.05),且观察组均低于对照组(P<0.05);两组JOA评分较治疗前上升(P<0.05),观察组高于对照组(P<0.05)。观察组总有效率为95.00%,高于对照组的82.50%(P<0.05)。两组治疗期间均无不良反应。结论活血止痛方联合活血通络腰椎巴布膏能够有效减轻腰椎间盘突出症急性期(气滞血瘀证)患者的临床症状,改善腰椎功能障碍,提升患者生活质量。Objective:To observe the clinical efficacy of Huoxue Zhitong Decoction combined with Huoxue Tongluo Lumbar Poultice in the treatment of lumbar disc herniation in the acute stage(syndrome of qi stagnation and blood stasis).Methods:A total of 80 patients with lumbar disc herniation in the acute staeg(syndrome of qi stagnation and blood stasis)were selected and divided into two groups with 40 cases in each group.On the basis of conventional treatment,the control group was given Huoxue Tongluo Lumbar Poultice,and the observation group was given Huoxue Zhitong Decoction combined with Huoxue Huoxue Tongluo Lumbar Poultice.Both groups were treated continuously for 10 days.The visual analogue score(VAS),Oswestry dysfunction(ODI)index,Japanese Orthopaedic Association score(JOA),TCM syndrome score,clinical efficacy and adverse reactions were compared before and after treatment.Results:After treatment,the scores of VAS,ODI and TCM syndrome(low back pain,numbness of lower limbs,stiffness of lumbar muscles,limited activity,fixed pain,and refusal to press on the painful area)in the two groups decreased compared with those before treatment(P<0.05),and the observation group was lower than that of the control group(P<0.05);the JOA scores in the two groups increased compared with those before treatment(P<0.05),and the observation group was higher than that in the control group(P<0.05).The total effective rate of the observation group(95.00%)was higher than that of the control group(82.50%)(P<0.05).No adverse reactions were reported during the treatment period in either group.Conclusion:Huoxue Zhitong Decoction combined with Huoxue Huoxue Tongluo Lumbar Poultice can effectively alleviate the clinical symptoms of patients with lumbar disc herniation in the acute stage(syndrome of qi stagnation and blood stasis),improve lumbar dysfunction,and improve the quality of life of patients.
