2017年至2022年四川省某三级甲等综合医院医院制剂使用现状及发展路径分析

摘　　要：目的为医院制剂的发展提供建议。方法利用医院信息系统对四川省某三级甲等综合医院2017年至2022年医院制剂使用数量、使用金额、制剂品种数等进行回顾性统计,分析该院医院制剂的使用特点,并为其发展提供建议。结果6年间,该院生产的西药制剂品种数由16种减至12种,中药制剂品种数一直保持为7种。医院制剂使用量总体呈增长趋势,但2020年和2022年同比前一年均有降低。西药制剂使用量构成比总体呈下降趋势,2017年至2019年的使用量及使用金额总体呈增长趋势,但2020年至2022年出现下滑,使用科室涉及皮肤科、血液内科、消化内科、内分泌科和五官科;中药制剂使用量构成比总体呈上升趋势,使用量及使用金额总体呈增长趋势,涉及皮肤科、消化内科、神经内科。两类制剂的使用均以皮肤科为主。结论该院西药医院制剂在6年间呈一定的萎缩现象,但仍有较大发展空间。医院应予以医院制剂支持,依规申请合理价格,拓展联盟医院调剂供给,增加网络物流配送业务等,以增强药品可及性。此外,还需以“患者需求”为导向创新制剂研发,助力医院制剂可持续发展。Objective To provide suggestions for the development of hospital preparations.Methods The amount,cost and variety of hospital preparations used in a tertiary-A general hospital in Sichuan from 2017 to 2022 were collected from the hospital information system.The characteristics of hospital preparations used were analyzed retrospectively,and suggestions for their development were provided.Results Sixteen varieties of western preparations produced by the hospital decreased to twelve varieties over the past six years,while there were always seven varieties of traditional Chinese preparations.The use amount of hospital preparations showed an overall increase,while there was a decrease in 2020 and 2022 compared to the previous year.The proportion of use amount of western preparations showed an overall decrease,the use amount and cost showed an overall increase from 2017 to 2019,while those showed a decrease from 2020 to 2022,involving departments of dermatology,hematology,gastroenterology,endocrinology and facial features.The proportion of use amount of traditional Chinese preparations showed an overall increase,as well as the use amount and cost,involving departments of dermatology,gastroenterology and neurology.The above two preparations were both mainly involved in the dermatological department.Conclusion The western hospital preparations used in this hospital show a certain decrease over the past six years,but there is still a large room for development.Hospitals should provide support for the hospital preparations,apply for rational price in accordance with regulations,expand the preparation supply of alliance hospitals,increase the online logistics delivery business to improve the accessibility of drugs.In addition,it is also necessary to focus on the preparation research and development guided by patients'needs to promote the sustainable development of hospital preparations.
