摘　　要：目的观察复元活血汤加减对pilon骨折患者术前肿痛及血清炎症因子的影响。方法选取2021年9月至2023年2月在我院治疗的62例pilon骨折患者,采用随机数字表法分成对照组和观察组各31例。两组患者入院后均予患肢跟骨牵引制动,对照组给予甘露醇静滴消肿治疗,观察组在此基础上联合复元活血汤加减治疗。对比治疗前后两组患肢肿胀率、疼痛视觉模拟量表(VAS)评分,对比两组血清中C反应蛋白(CRP)、肿瘤坏死因子-(TNF-α)、白介素-6(IL-6)水平,比较两组临床疗效。结果治疗后,观察组患肢肿胀率、VAS评分低于对照组,差异有统计学意义(P<0.01)。治疗后,患者血清CRP、TNF-α、IL-6水平低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组总有效率为93.5%,显著高于对照组的58.1%,差异有统计学意义(P<0.01)。结论复元活血汤加减能有效减轻pilon骨折患者术前肿胀及疼痛症状,降低血清炎症因子的水平,值得在临床上推广。Objectives To observe the effect of Fuyuan Huoxuetang on preoperative swelling and pain and serum inflammatory factors in patients with pilon fractures.Methods A total of 62 patients with pilon fractures treated in our hospital from September 2021 to February 2023 were selected.The patients were divided into control group and observation group according to the random number table method,with 31 cases in each group.Two groups of patients were treated with calcaneal traction of affected limbs to limit movement after admission,the control group was treated with mannitol intravenous infusion for detumescence,on which basis the observation group was treated with Fuyuan Huoxuetang.The limb swelling rate,visual analogue scale(VAS)scores,C-reactive protein(CRP),tumor necrosis factor-(TNF-α),and Interleukin-6(IL-6)in the serum of the two groups,and the clinical efficacy were compared.Results After treatment,the swelling rate and VAS scores of the affected limb in the observation group were significantly lower than those in the control group(P<0.01).After treatment,the levels of serum CRP,TNF-αand IL-6 in the patients were significantly lower than those in the control group(P<0.05).After treatment,the total effective rate of the observation group was 93.5%,which was significantly higher than the 58.1%in the control group(P<0.01).Conclusions Fuyuan Huoxuetang can effectively relieve preoperative swelling and pain symptoms of patients with pilon fractures,and reduce the level of serum inflammatory factors,which is worthy of clinical promotion.
