降压益肾颗粒联合氨氯地平治疗老年原发性高血压的疗效及对血清糖化血红蛋白及肾胺酶水平的影响

摘　　要：目的:探讨降压益肾颗粒联合氨氯地平对老年原发性高血压患者的临床效果及对血清糖化血红蛋白(HbA1c)和肾胺酶(Renalase蛋白)水平的影响。方法:选择在我院接受治疗的125例原发性高血压老年患者为研究对象,依据随机数字表法分为氨氯地平组(40例)、降压益肾颗粒组(40例)及联合治疗组(45例,采用氨氯地平和降压益肾颗粒联合治疗)。各组均给予相应的治疗,8周后,观察比较各组患者降压效果,检测各组治疗前后血清renalase蛋白和HbA1c水平变化。结果:联合治疗组总有效率显著高于氨氯地平组和降压益肾颗粒组(88.9%比65.0%比70.0%),P<0.05或<0.01;治疗后,联合治疗组血压变异性指标较治疗前及氨氯地平组和降压益肾颗粒组均有显著改善(P<0.05或<0.01);治疗后,与氨氯地平组和降压益肾颗粒组比较,联合治疗组renalase蛋白显著升高[(1380.17±120.13)pg/ml比(1480.17±110.93)pg/ml比(1845.43±112.38)pg/ml],HbA1c[(5.92±0.35)%比(5.83±0.36)%比(5.31±0.35)%]水平显著降低(P均=0.001)。结论:降压益肾颗粒联合氨氯地平治疗老年原发性高血压患者,可加强降压疗效,降低HbA1c水平、升高renalase蛋白水平,使患者获益。Objective:To investigate the therapeutic effect of Jiangya Yishen granules(JYG)combined with amlodipine on elderly patients with essential hypertension(EH)and its impact on serum levels of glycosylated hemoglobin(HbA1c)and renalase protein.Methods:A total of 125 elderly EH patients treated in our hospital were selected.According to random number table,they were divided into amlodipine group(n=40),JYG group(n=40)and combined treatment group(n=45,received amlodipine combined with JYG).Each group received corresponding treatment for eight weeks.Therapeutic effect,antihypertensive effect,serum levels of renalase protein and HbA1c were compared among three groups before and after treatment.Results:Total effective rate of combined treatment group was significantly higher than those of amlodipine group and JYG group(88.9%vs.65.0%vs.70.0%),P<0.05 or<0.01;compared with amlodipine group and JYG group after treatment,there were significant improvements in blood pressure variability indexes(P<0.05 or<0.01),significant rise in renalase protein level[(1380.17±120.13)pg/ml vs.(1480.17±110.93)pg/ml vs.(1845.43±112.38)pg/ml],and significant reduction in HbA1c level[(5.92±0.35)%vs.(5.83±0.36)%vs.(5.31±0.35)%]in combined treatment group,P=0.001 all.Conclusion:Jiangya Yishen granules combined with amlodipine can strengthen antihypertensive effect,reduce HbA1c level and increase renalase protein level in elderly patients with essential hypertension,which can benefit the patients.
