口服华法林致牙龈出血患者的风险防范与口腔护理

摘　　要：目的观察中药口腔护理液在口服华法林致牙龈出血患者中的应用效果。方法选取在心内科住院的口服华法林进行治疗的150例患者作为研究对象,随机分为对照1组、对照2组和实验组,每组50例。3组患者均接受华法林用药护理及口腔卫生指导,对照1组使用生理盐水漱口,对照2组使用银离子漱口液漱口,实验组使用中药口腔护理液漱口。干预后,观察并比较3组患者牙龈出血次数、龈沟出血指数(SBI)和口臭评分。结果实验组患者漱口期间(10 d)、出院4周内牙龈出血次数均少于对照1组、对照2组,差异有统计学意义(P<0.05);实验组患者漱口期间SBI为(1.53±0.69)分,低于对照1组的(2.12±0.79)分、对照2组的(2.09±0.76)分,差异有统计学意义(P<0.05);实验组患者漱口期间口臭评分为(1.70±0.68)分,低于对照1组的(2.28±0.74)分、对照2组的(2.23±0.79)分,差异有统计学意义(P<0.05)。结论口服华法林致牙龈出血患者使用中药口腔护理液漱口,能够明显减少牙龈出血次数,降低SBI,减轻口臭,改善口腔卫生状况,提高用药安全性。Objective To observe effect of TCM oral care solution in patients with gingival bleeding caused by oral administration of warfarin.Methods A total of 150 hospitalized patients in the cardiology department were selected as study objects,and were randomly divided into control group 1,control group 2 and experimental group,with 50 patients per group.Patients in the three groups were given medication care of warfarin and health guidance for mouth care.The control group 1 increased the use of normal saline gargle;the control group 2 increased the use of silver ion gargle,and the experimental group was given the use of traditional Chinese medicine oral care solution gargle.After the intervention,the number of gingival bleeding,gingival sulcus bleeding index(SBI)and halitosis score in the three groups were compared.Results The number of gingival bleeding during gargling(10 d)and within 4 weeks after discharge in the experimental group were less than that in the control group 1 and control group 2,the difference was statistically significant(P<0.05).The SBI of the experimental group was(1.53±0.69),which was lower than(2.12±0.79)in the control group 1 and(2.09±0.76)in the control group 2(P<0.05).The score of halitosis during gargling in the experimental group was(1.70±0.68),which was lower than(2.28±0.74)in the control group 1 and(2.23±0.79)in the control group 2(P<0.05).Conclusion The TCM oral care solution for patients with gingival bleeding caused by oral administration of warfarin can significantly reduce the number of gingival bleeding,reduce SBI,alleviate bad breath,improve oral health and drug safety.
