摘 要:目的探讨气动牙挺微创拔牙在即刻种植修复中的应用效果。方法选取2020年1月-2023年1月广州市中西医结合医院口腔颌面外科门诊收治的90例诊断为前牙残根且有种植需求的患者作为研究对象,随机分为对照组和研究组,对照组(45例)采用传统牙挺微创拔除残根,研究组(45例)采用气动牙挺微创拔除残根。比较两组患者患牙拔除后牙槽骨损伤程度、进行即刻种植修复后的美学效果以及术中软组织损伤与术后并发症发生情况。结果拔牙后研究组4mm、7mm处牙槽骨宽度差值以及高度差值均低于对照组(P<0.05);研究组近中龈乳头、远中龈乳头、牙龈边缘水平、牙龈外形、牙龈颜色、牙龈质地、牙槽嵴缺损评分均高于对照组(P<0.05);研究组术中软组织损伤与术后并发症发生率(6.67%)低于对照组(28.89%)(P<0.05)。结论气动牙挺微创拔牙可减轻对牙槽骨的损伤,提高即刻种植后的美学效果,术中软组织损伤与术后并发症发生率较低,可将气动牙挺微创拔牙术用于即刻种植中,为即刻种植提供优良的种植条件。Objective To investigate the clinical effect of micro-invasive pneumatic tooth extraction in immediate implant restoration.Methods A total of 90 patients who were diagnosed as having anterior residual roots and needed dental implants from January 2020 to January 2023 were randomly divided into control group and study group,the control group(45 cases)was treated with traditional tappet and the study group(45 cases)was treated with pneumatic tappet.The degree of alveolar bone injury,the aesthetic effect after immediate implant restoration and the occurrence of soft tissue injury and postoperative complications were compared between the two groups.Results The differences of alveolar bone width and height at 4mm and 7mm were lower in the study group than in the control group(P<0.05).The scores of gingival papilla,gingival marginal level,gingival shape,gingival color,gingival texture and alveolar ridge defect in the study group were higher than those in the control group(P<0.05).The incidence of intraoperative soft tissue injury and postoperative complications in the study group(6.67%)was lower than that in the control group(28.89%)(P<0.05).Conclusion Micro-invasive extraction with pneumatic tappet can reduce the damage to alveolar bone and improve the aesthetic effect after immediate implantation,the utility model can apply the air-operated tooth-lifting technique in the immediate implantation,and provides excellent implantation conditions for the immediate implantation.
