摘 要:目的 分析高尿酸人群平均动脉压(MAP)水平与踝臂脉搏波传导速度(baPWV)之间的相关性。方法 选择在开滦总医院等11家医院2006—2007年、2008—2009年、2010—2011年、2012—2013年、2014—2015年、2016—2017年健康体检并完成baPWV检测的受检者9 020例,采用偏相关分析baPWV与MAP的相关性,用多元线性回归分析、多因素logistic回归分析MAP与baPWV的关系。结果 高尿酸人群中随MAP水平增加,baPWV水平也增加。在调整了年龄后,MAP与baPWV呈正相关(r=0.367,P<0.001)。多元线性回归分析结果显示,调整年龄、性别等危险因素后,MAP与baPWV呈正相关(P<0.001)。按照MAP进行三分位组及按性别分层也得出相似结果。校正其他混杂因素后进行多因素logistic回归分析表明,MAP是baPWV≥1 400 cm/s的危险因素(OR=6.639,P<0.001)。结论 高尿酸人群中MAP水平与baPWV呈正相关,MAP升高是动脉硬化的危险因素,女性高尿酸患者MAP升高促动脉硬化的进展更为明显。Objective To analyze the correlation between the mean arterial pressure(MAP)level and ankle artery pulse wave(baPWV)in the hyperuricemic population.Methods Nine thousand and twenty cases were selected to participate in health examinations and complete baPWV testing in 11 hospitals including Kailuan General Hospital(2006-2007,2008-2009,2010-2011,2012-2013,2014-2015,2016-2017).The correlation between baPWV and MAP was analysed using partial correlation,multiple linear regression analysis and multi-factor logistic regression to analyse the effect of MAP on baPWV.Results BaPWV levels increased with increasing MAP levels in hypertensive population.After adjusting for age,MAP levels were positively correlated with baPWV(r=0.367,P<0.001).Multiple linear regression analysis showed a positive correlation relationship between MAP levels baPWV after adjusting for risk factors such as age and gender(P<0.001).Similar results were obtained for trichotomous groups according to MAP levels and stratification by sex.Multi-factor logistic regression analysis after correcting for other confounding factors showed that MAP was a risk factor for increased baPWV≥1400 cm/s(OR=6.639,P<0.001).Conclusions MAP levels in the hyperuricemic population were positively correlated with baPWV,and elevated MAP is a risk factor for atherosclerosis,and elevated MAP promotes the progression of atherosclerosis more significantly in female patients with high serum uric acid.
