摘　　要：目的:探讨分析Wiltse入路椎弓根螺钉内固定治疗胸腰椎骨折的效果。方法:将苏州市相城人民医院2020年1月—2022年1月收治的60例胸腰椎骨折患者按随机数字表法分为Wiltse入路组(A组,n=30)和后路椎旁入路组(B组,n=30)。比较两组手术总时长、总失血量、术后首次下床活动时间、住院总天数;比较两组术前和术后1周、12个月的视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)、伤椎前后缘高度比及Cobb角。结果:A组手术总时长、术后首次下床活动时间、住院总天数均较B组更短,总失血量较B组更少(P<0.05)。A组术后1周的VAS评分、ODI均较B组明显更低(P<0.05)。两组术后1周及12个月伤椎前后缘高度比均较治疗前增高,Cobb角均较术前减小(P<0.05),但两组间比较,差异均无统计学意义(P>0.05)。结论:Wiltse入路椎弓根钉内固定治疗胸腰椎骨折具有手术时间短、失血少、损伤小、恢复快的优点。Objective:To investigate and analyze the effect of pedicle screw internal fixation through Wiltse approach in the treatment of thoracic and lumbar spine fracture.Method:A total of 60 patients with thoracic and lumbar spine fracture admitted to Suzhou Xiangcheng People's Hospital from January 2020 to January 2022 were divided into Wiltse approach group(group A,n=30)and posterior paravertebral approach group(group B,n=30)according to random number table method.The total operation time,total blood loss,first time of getting out of bed after operation and total hospitalization days were compared between the two groups.Visual analogue scale(VAS)scores,Oswestry disability index(ODI),anterior and posterior edge height ratio of injured vertebra and Cobb angle were compared between the two groups before operation and 1 week and 12 months after operation.Result:The total operation time,first time of getting out of bed after operation and total hospitalization days in group A were shorter than those in group B,and total blood loss in group A was less than that in group B(P<0.05).1 week after operation,VAS score and ODI in group A were significantly lower than those in group B(P<0.05).1 week and 12 months after operation,anterior and posterior edge height ratio of injured vertebra in both groups were higher than those before operation,and the Cobb angle in both groups were lower than those before operation(P<0.05),but there were no statistical significances between the two groups(P>0.05).Conclusion:Wiltse approach in the treatment of thoracic and lumbar spine fracture has the advantages of short operation time,less blood loss,less injury and faste r recovery.
