摘 要:目的探讨宁夏食品及病例中单核细胞增生李斯特菌流行特征及耐药现状。方法对宁夏2006—2020年食品及病例中分离的138株李斯特菌采用肉汤稀释法、血清凝集和脉冲场凝胶电泳(PFGE)进行药敏、血清及分子分型研究。结果食品来源113株菌共分属6个血清型,优势血清型为1/2a型,共45株,占39.82%。其次为1/2c型,共32株,占28.32%,致病性较强的4b型在食品中亦分离到5株,占4.42%。25株病例来源菌株分属4个血清型,优势血清型为致病性较强的4b型,共12株,占48.00%;138株不同来源李斯特菌对8种抗生素均出现不同程度的耐药;食品来源菌株共分属17个耐药谱,病例来源菌株分属7个耐药谱,优势耐药谱一致,均为VAN-TET。电泳后,113株食品来源分离株获得94个不同的PFGE带型,相似度为37.1%~97.0%。25株病例来源菌株共分为22个不同的PFGE带型,相似度为46.2%~100%。不同时期、不同来源、不同地区分离株PFGE带型优势型别不明显,呈散在分布。结论宁夏单核细胞增生李斯特菌血清型呈现多元化流行趋势,不同地区流行特征呈现多样性。本地区单核细胞增生李斯特菌病应采取区域化防治手段,临床治疗尽可能在药敏结果的指导下合理用药。Objective To investigate the epidemic characteristics and drug resistance of Listeria monocytogenes in food and cases in Ningxia.Methods The sensitivity,serum and molecular typing of 138 strains of Listeria monocytogenes isolated from food and cases in Ningxia from 2006 to 2020,which were analyzed by broth dilution,serum agglutination and pulsed field gel electrophoresis(PFGE).Results The 113 strains from food were classified into 6 serotypes.The dominant serotype was 1/2a with totally of 45 strains,which accounting for 39.82%.The second type was 1/2c with 32 strains that accounting for 28.32%.Type 4b with strong pathogenicity was also isolated from food that accounting for 4.42%.The 25 strains of case origin belonged to 4 serotypes,and the dominant serotype was type 4b with strong pathogenicity which accounting for 48.00%and having totally 12 strains.The 138 strains of Listeria monocytogenes from different sources showed different degrees of resistance to 8 antibiotics.The food derived strains belonged to 17 drug resistance spectra,and the case derived strains belonged to 7 drug resistance spectra with the same dominant drug resistance spectra,all of which were VAN-ETE.After gel electrophoresis,113 strains of food isolates obtained 94 different PFGE bands,with similar of 37.1%-97.0%.The 25 strains were divided into 22 different PFGE bands,and with similar of 46.2%-100%.The dominant PFGE bands of isolates from different periods,different sources and different regions were not obvious,but scattered.Conclusion The serotypes of Listeria monocytogenes showed a diversified epidemic trend in Ningxia,and the flow characteristics in different regions showed diversely.The regional prevention and treatment of Listeria monocytogenes in this region should be taken,and the clinical treatment should be rationally under the guidance of drug sensitivity results as far as possible.
