张景岳“温理脾肾之阴”治痢思想探析

摘　　要：张景岳强调“治痢”当辨虚实、寒热,在治疗脾肾中虚型痢疾时,提出温理脾肾之阴,滋补精血,涩肠止痢,达到治疗目的。本文梳理张氏“阳非有余论”及“真阴论”,总结其治痢思想;针对脾肾中虚型痢证,予理阴煎,以熟地、当归理脾肾之阴,滋补精血,辅干姜、肉桂温脾肾之阳;及后世医者仿理阴煎治疗该证型时,温理脾肾之阴,兼顾治表。以期为现代临床治疗痢疾提供新思路、新方法。Zhang Jingyue emphasized that“Treating Dysentery”should distinguish between deficiency and excess,cold and heat.When treating dysentery of deficiency type in spleen and kidney,he proposed to warm and regulate the Yin of spleen and kidney,nourish essence and blood,astringe intestines and stop dysentery,so as to achieve the objective of treatment.The author combs Zhang’s“Yang is not surplus”and“true Yin”and summarizes his thought of treating dysentery;According to the dysentery syndrome of deficiency in spleen and kidney,Liyin decoction is given,cooked earth and Angelica are used to regulate the Yin of spleen and kidney,nourish essence and blood,supplemented by dried ginger and cinnamon to warm the Yang of spleen and kidney;And later doctors who imitated Liyin decoction to treat this syndrome type,warming and regulating the Yin of spleen and kidney,taking into account the treatment of surface to provide new ideas and methods for modern clinical treatment of dysentery.
