以结局为导向的多元化康复护理对大面积烧伤患者创面愈合和外观接受度的影响

摘　　要：目的:探讨以结局为导向的多元化康复护理对大面积烧伤患者创面愈合和外观接受度的影响。方法:选取2022年1月-2022年12月笔者医院收治的大面积烧伤患者101例,按简单随机分组法将入组患者分为观察组(n=51)和对照组(n=50),对照组给予常规护理干预,观察组在对照组基础上给予以结局为导向的多元化康复护理,均持续护理至患者出院。比较两组创面愈合时间。于护理前后,比较两组心理弹性[心理韧性量表(Chinese version of resilience scale,CD-RISC)]、病耻感[社会影响量表(Social impact scale,SIS)]、社会支持度[社会支持评定量表(Social support revalued scale,SSRS)]、外观接受度[伤残接受度量表(Acceptance of disabilityscal,AODS)]和应对方式[心理特征应对量表(Trait coping style questionnaire,TCSQ)]评分。结果:观察组创面愈合时间显著短于对照组(P<0.05)。护理后,观察组坚韧性、力量性和乐观性等CD-RISC评分均高于对照组(P<0.05);观察组社会排斥、内在羞耻感、社会隔离和经济歧视等SIS评分均显著低于对照组(P<0.05);观察组AODS、SSRS和积极应对评分均显著高于对照组,消极应对评分低于对照组,差异均有统计学意义(P<0.05)。结论:以结局为导向的多元化康复护理有利于提高大面积烧伤患者心理弹性、外观接受度和社会支持水平,降低病耻感,改变应对方式,促使创面愈合。Objective To explore the effects of outcome-oriented diversified rehabilitation nursing on wound healing and appearance acceptance in patients with extensive burn.Methods A total of 101 patients with extensive burn admitted to the author's hospital were enrolled between January and December 2022.According to simple random grouping method,they were divided into observation group(n=51)and control group(n=50).The control group was given routine nursing,while observation group was additionally given outcome-oriented diversified rehabilitation nursing.All were continuously nursed till discharge.The wound healing time,psychological resilience[Chinese version of resilience scale(CD-RISC)],stigma[Social Impact Scale(SIS)],social support[Social Support Rating Scale(SSRS)],appearance acceptance[acceptance of disability(AODS)]and coping styles[trait coping style questionnaire(TCSQ)]scores of the two groups were compared before and after intervention.Results The wound healing time in observation group was significantly shorter than that in control group(P<0.05).After nursing,scores of CD-RISC(toughness,strength,optimism)in observation group were higher than those in control group(P<0.05),and scores of SIS(social exclusion,intrinsic shame,social isolation,economic discrimination)were significantly lower than those in control group(P<0.05).The scores of AODS,SSRS and positive coping in observation group were significantly higher than those in control group,while score of negative coping was lower than that in control group(P<0.05).Conclusion Outcome-oriented diversified rehabilitation nursing is beneficial to improve psychological resilience,appearance acceptance and social support level,reduce stigma,change coping styles and promote wound healing in patients with extensive burns.
