某院静脉用药调配中心退药情况及对策分析

摘　　要：目的分析静脉用药调配中心(PIVAS)药品退药情况,优化退药流程,提高PIVAS的退药工作效率和质量。方法收集我院2022年1月至2022年12月通过PIVAS配置的所有医嘱,统计其中的退药数量、科室、批次、种类,分析原因并提出对策。结果2022年1月至2022年12月通过PIVAS配置的药品医嘱1046235条,其中退药医嘱共计78696条,退药率为7.52%。退药率较高的科室有胸外科(12.23%)、重症室(11.48%),退药率最高的用药批次是第四批药物(12.80%),退药率最高的药品种类是全胃肠外营养液(12.96%)。结论目前我院PIVAS的退药率较高,应针对性进行退药流程优化,同时降低住院患者PIVAS配置药品的退药率,提高PIVAS的退药工作效率,减少退回差错,保障用药安全。OBJECTIVE Analyze the drug withdrawal situation in the Pharmacy Intravenous Admixture Services(PIVAS),optimize the withdrawal process,and improve the efficiency and quality of the withdrawal process in PIV⁃AS.METHODS We collected all medical orders configured through PIVAS in our hospital from January 2022 to December 2022,counted the number,departments,batches,and types of returned drugs in them,analyzed the reasons and proposed countermeasures.RESULTS There were 1046,235 medication orders configured through PIVAS from January 2022 to December 2022,of which a total of 78,696 were returned,with a return rate of 7.52%.The depart⁃ments with high withdrawal rates were thoracic surgery(12.23%)and intensive care unit(11.48%),the medica⁃tion batch with the highest withdrawal rate was the fourth batch of medication(12.80%),and the medication type with the highest withdrawal rate was total parenteral nutrition solution(12.96%).CONCLUSION Currently,the withdrawal rate of PIVAS in our hospital is high,so we should target the optimization of the withdrawal process,and at the same time,reduce the withdrawal rate of inpatient PIVAS⁃configured drugs,improve the withdrawal efficiency of PIVAS,reduce return errors,and guarantee the safety of medication use.
