包皮作为移植材料在阴茎硬结症手术中的应用

摘　　要：目的探讨包皮作为移植材料在阴茎硬结症手术中的疗效。方法选取2018年1月至2020年8月本院收治的24例阴茎硬结症患者,采用阴茎硬结切除术+包皮补片修补术进行治疗,比较患者术前和术后1年的国际勃起功能指数-5(IIEF-5)问卷评分以及阴茎彩色双功能多普勒检查(CDDU)结果。结果24例患者的术后阴茎弯曲畸形均得到明显好转,能进行正常性生活,无阴茎勃起疼痛,阴茎长度无缩短,术后1年的IIEF-5评分高于术前[(20.96±0.27)分vs.(19.08±0.43)分],差异有统计学意义(P<0.05);CDDU检查结果显示,患者术前的收缩期峰值流速(PSV)、舒张末期速度(EDV)与术后比较,差异均无统计学意义(P=0.390、0.830)。结论包皮作为移植材料治疗阴茎硬结症是一种安全有效且简单经济的手术方式,但仍需更多病例进行长期、前瞻性、随机对照研究来进一步评估其治疗效果。Objective To investigate the effect of foreskin as graft material in the operation of Peyronie′s disease.Methods From January 2018 to August 2020,24 patients with peyronie′s disease were treated with plaque excision and corporoplasty with foreskin graft.IIEF-5(international index of erectile function-5)questionnaire score and color duplex doppler ultrasonography(CDDU)were performed before and 1 year after operation.Results Penile curvature was obviously relieved after operation,all of them could have normal sexual life,no penile erection pain,no penile shortening was observed.The IIEF-5 score one year after operation was significantly higher than that before operation[(20.96±0.27)scores vs.(19.08±0.43)scores,P<0.05].CDDU showed that there was no significant difference in peak systolic velocity(PSV)and end-diastolic velocity(EDV)between patients before and after surgery(P=0.390,0.830).Conclusions Corporoplasty with foreskin as graft material is a safe,simple and effective option for the treatment of Peyronie′s disease.But long-term prospective,randomized studies with higher numbers of patients are warranted to further investigate this treatment method.
