叶绿素衍生物对痔术后创面愈合的促进作用

摘　　要：目的探讨叶绿素衍生物对痔术后创面愈合的影响。方法选取行吻合器痔上黏膜环切术(PPH)的环形痔术后患者124例,分为对照组和试验组。比较2组肉芽形成时间、创面愈合时间,治疗前后创面水肿和疼痛程度,血清生化指标,不良反应和并发症。结果试验组肉芽形成时间、创面愈合时间均短于对照组,治疗后2组创面水肿和疼痛评分、血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)水平均降低,且治疗后试验组均低于对照组,试验组上述指标治疗前后差值均高于对照组,治疗后2组血清表皮生长因子(EGF)、纤维连接蛋白(FN)、碱性成纤维细胞因子(bFGF)水平均升高,且治疗后试验组均高于对照组,差异有统计学意义(P<0.05)。结论叶绿素衍生物可促进痔术后肉芽形成与创面愈合,减轻患者水肿和疼痛。Objective To explore the effect of chlorophyll derivative on wound healing after hemorrhoidectomy.Methods A total of 124 patients with circular hemorrhoids after procedure for prolapse and hemorrhoids(PPH)were selected and divided into control group and experimental group.The granulation formation time,wound healing time,degree of wound edema and pain before and after treatment,serum biochemical indicators,adverse reactions and complications were compared between two groups.Results The granulation formation time and wound healing time in the experimental group were significantly shorter than those in the control group(P<0.05).After treatment,wound edema score and pain score as well as serum levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in both groups were significantly lower than those before treatment,and these indexes after treatment in the experimental group were significantly lower than those in the control group(P<0.05),but the differences of the above indexes before and after treatment in the experimental group were significantly higher than those in the control group(P<0.05).After treatment,the levels of serum epidermal growth factor(EGF),fibronectin(FN)and basic fibroblast factor(bFGF)in both groups increased significantly,and these indexes after treatment in the experimental group were significantly higher than those in the control group(P<0.05).Conclusion Chlorophyll derivative can promote granulation formation and wound healing after hemorrhoidectomy,and reduce degrees of edema and pain.
