当归芍药散治疗原发性痛经的临床疗效

摘　　要：目的:探讨当归芍药散治疗原发性痛经(PD)的临床疗效。方法:选取符合纳入标准的气滞血瘀型PD患者72例作为研究对象,按随机数字法随机分为观察组和对照组,每组36例。观察组给予当归芍药散治疗;对照组给予元胡止痛片治疗。2组患者均于经前1周开始服药,10 d为1个疗程,共3个疗程。观察2组治疗前后中医证候积分、血清前列腺素F_(2α)(PGF_(2α))及环氧合酶2(COX-2)的变化。结果:治疗后观察组与对照组的总有效率分别为97.14%、63.64%,差异有统计学意义(P<0.05);观察组患者中医证候积分低于对照组,差异有统计学意义(P<0.05);观察组患者血清PGF_(2α)及COX-2水平低于对照组,差异均有统计学意义(均P<0.05)。结论:当归芍药散能够缓解PD的症状,通过抑制COX-2表达,降低PGF_(2α)的水平,且临床无不良反应。Objective:To explore the clinical efficacy of Danggui Shaoyao Powder in the treatment of primary dysmenorrhea(PD).Methods:A total of 72 PD patients with qi stagnation and blood stasis,meeting the inclusion criteria,were randomly divided into observation group and control group,with 36 patients in each group.The observation group was treated with Danggui Shaoyao Powder and the control group was treated with Yuanhu Zhitong Tablets.The patients in both groups started medication one week before menstruation,with each course lasting for 10 days and a total of 3 courses.The changes in traditional Chinese medicine(TCM)syndrome scores,serum prostaglandinF_(2α)(PGF_(2α))and cyclooxygenase 2(COX-2)were observed before and after treatment in both groups.Results:After treatment,the total effective rate in the observation group and control group was 97.14%and 63.64%,respectively,with statistical significance(P<0.05).The TCM syndrome scores of the observation group after treatment were lower than those of the control group,and the differences were statistically significant(P<0.05).The levels of serum PGF_(2α)and COX-2 in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Danggui Shaoyao Powder can relieve the symptoms of PD by inhibiting the expression of COX-2 and reducing the level of PGF_(2α)without causing adverse reactions in clinic.
