李世增教授隔脏治疗法论治慢性胃炎的诊治规律及用药特点

摘　　要：目的:通过对临床病案进行数据挖掘,从客观角度,验证及分析李世增教授运用隔脏治疗法治疗慢性胃炎的诊治规律及用药特点。方法:借助“中医传承计算平台V3.0”软件,对2017年12月至2021年3月李世增老师门诊治疗慢性胃炎的207个处方进行数据挖掘,主要运用软件中“数据分析”模块中“统计分析”“方剂分析”“症状分析”功能,进行治法、用药及其组方规律分析。并基于药物关联规则分析展示药物关联规则网络视图。结果:所有处方涉及治法40个,前5位按频次由多至少依次为健脾、和胃、疏肝、清化、养心。所有治法按涉及脏腑统计,14个治法涉及脏腑,统计其包含处方数发现从脾、胃论治最多,其次是从肝论治。207个处方中包含146味中药,用药频次≥50次的药物34个,其中炙甘草、连翘、太子参、白术、蒲公英、丹参、白豆蔻、佛手、延胡索、白芍使用频次>100次。处方中药物按四气分布以寒性药物最多;五味分布苦味最多;按归经分布以脾经最多;按药物功效分布前5类依次是补虚类、理气类、清热类、活血化瘀类、利水渗湿类。结论:李世增老师治疗慢性胃炎,以清法贯穿始终,归肺经、肝经药排第二、第三,体现“隔脏治疗法”选药特点,常用健脾、理气、清降的药物,体现“隔脏治疗法”治疗慢性胃炎的学术思想。Objective:To objectively verify and analyze the diagnosis and treatment rules and prescription characteristics of Professor LI Shizeng in the treatment of chronic gastritis with cross viscera therapy through data mining of clinical cases.Methods:A total of 207 prescriptions for the treatment of chronic gastritis by Professor LI Shizeng in the department from December 2017 to March 2021 were collected using the Traditional Chinese Medicine Inheritance Calculation Platform V 3.0.TheStatistical Analysis,Prescription Analysis,and Symptom Analysis functions in the Data Analysis module of the Traditional Chinese Medicine Inheritance Calculation Platform V3.0 were employed to analyze the treatment methods as well as the prescriptions and the rules.Additionally,association rules were used for network visualization of Chinese medicines.Results:Forty treatment methods were involved in all prescriptions,and the top five by frequency were spleen invigorating,stomach harmonizing,liver soothing,clearing and resolving,and heart nourishing.According to the zang-fu organs involved,there were 14 treatment methods.By analyzing the number of the prescriptions,it was found that the treatment was most carried out from the spleen and stomach,followed by the liver.There were 207 prescriptions including 146 Chinese medicines,and there were 34 medicines with the usage of frequency≥50,among which,Radix et Rhizoma Glycyrrhizae Praeparata Cum Melle,Fructus Forsythiae,Radix Pseudostellariae,Rhizoma Atractylodis Macrocephalae,Herba Taraxaci,Radix et Rhizoma Salviae Miltiorrhizae,Fructus Amomi Rotundus,Fructus Citri Sarcodactylis,Rhizoma Corydalis,and Radix Paeoniae Alba were used more than 100 times.Medicines with cold nature and those with bitter flavor were found most,and they mostly acted on the spleen.According to the efficacy,the top five prescriptions were those for tonifying,regulating qi,clearing heat,activating blood and resolving stasis,and promoting urination and draining dampness.Conclusion:Professor Li uses the cleaning method t
