透皮疗法联合综合康复护理在类风湿性关节炎患者中的应用

摘　　要：目的探讨类风湿性关节炎(RA)患者采用透皮疗法联合综合康复护理的效果。方法选取2020年2月至2022年9月在河南省中西医结合医院接收的RA患者共计136例,以随机数字表法分为干预组与对照组,各68例。对照组实行常规康复护理干预,干预组实行透皮疗法联合综合康复护理干预,对两组膝关节功能、疼痛程度、心理状态、生活质量进行比较。结果与同组干预前相比,两组干预后美国纽约特种外科医院(HSS)膝功能评分量表评分均提高,且干预组较对照组更高,差异均有统计学意义(P<0.05);与同组干预前相比,两组干预后视觉模拟评分法(VAS)评分均下降,且干预组较对照组更低,差异均有统计学意义(P<0.05);与同组干预前相比,两组干预后汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)评分均降低,且干预组较对照组更低,差异均有统计学意义(P<0.05);与同组干预前相比,两组干预后世界卫生组织生存质量测定量表简表(WHOQOL-BREF)评分提高,且干预组较对照组更高,差异均有统计学意义(P<0.05)。结论透皮疗法联合综合康复护理能够增强患者的膝关节功能,改善患者疼痛程度及心理状态,同时提升患者生活质量。Objective To explore the effect of transdermal therapy combined with comprehensive rehabilitation nursing for patients with rheumatoid arthritis(RA).Methods A total of 136 RA patients admitted to Henan Integrated Traditional Chinese and Western Medicine Hospital from February 2020 to September 2022 were selected and randomly divided into an intervention group and a control group,with 68 cases in each group,using a random number table method.The control group received routine rehabilitation nursing intervention,while the intervention group received transdermal therapy combined with comprehensive rehabilitation nursing intervention.The knee joint function,pain degree,psychological status and quality of life of the two groups were compared.Results Compared with before the intervention in the same group,both groups showed an improvement in the knee function score scale scores of the New York Special Surgery Hospital(HSS)in the United States after the intervention,and in comparision of the control group,the intervention group was higher,with statistical significance(P<0.05).After intervention,the visual analogue scale(VAS)scores in both groups were decreased,and in comparision of the control group,the intervention group was lower,with statistical significance(P<0.05).After intervention,the scores of Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)in both groups were decreased,and in comparision of the control group,the intervention group was lower,with statistical significance(P<0.05).Compared with before intervention in the same group,the World Health Organization Quality of Life Scale Brief(WHOQOL-BREF)scores improved in both groups after intervention,,and in comparision of the control group,the intervention group was higher,with statistical significance(P<0.05).Conclusion Transdermal therapy combined with comprehensive rehabilitation care can enhance the knee joint function of patients,improve their pain level and psychological state,and improve their quality of life.
