摘  要:目的:探讨动态增强磁共振成像(DCE-MRI)血流动力学参数在高、低级别胶质瘤鉴别中的应用,分析其与分子生物学标记物水平的相关性。方法:选取86例脑胶质瘤患者为研究对象。以病理结果作为金标准,按WHO肿瘤分类标准,将其分为低级别组(n=41)、高级别组(n=45),获得常规和动态增强MRI扫描参数数值,通过受试者工作特征曲线(ROC)分析DCE-MRI血流动力学参数对高、低级别胶质瘤的鉴别价值,Pearson分析各血流动力学参数与癌组织分子生物学标志物的相关性。结果:高级别组DCE-MRI血流动力学参数Ktrans、Kep、Ve水平均高于低级别组(P<0.05);高级别组与低级别组Vp水平比较,差异无统计学意义(P>0.05);DCE-MRI血流动力学参数Ktrans、Kep、Ve联合检测鉴别高、低级别脑胶质瘤ROC曲线下面积(AUC)为0.927,高于单独检测AUC(0.822、0.803、0.785),敏感度为91.11%、特异度为80.49%(P<0.05);高级别组脑胶质瘤分子生物学标记物VEGF mRNA、Ki-67 mRNA表达水平高于低级别组(P<0.05);Pearson相关性分析显示,脑胶质瘤DCE-MRI血流动力学参数Ktrans、Kep、Ve水平与癌组织VEGF mRNA、Ki-67 mRNA表达水平正相关(r=0.522、0.415、0.323、0.483、0.376、0.274,P<0.05)。结论:高级别脑胶质瘤DCE-MRI血流动力学参数Ktrans、Kep、Ve水平均高于低级别胶质瘤,且与癌组织分子生物学标记物VEGF、Ki-67表达水平正相关,可用于临床高、低级别脑胶质瘤鉴别。Objective:To explore the application of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)hemodynamic parameters in the identification of high and low grade gliomas,and to analyze its correlation with molecular biological markers.Methods:The clinical data of 86 patients with glioma were retrospectively analyzed.The pathological results were used as the gold standard.According to the WHO tumor classification criteria,they were divided into low-grade group(n=41)and high-grade group(n=45).The values of conventional and dynamic enhanced MRI scan parameters were obtained.The receiver operating curve(ROC)was used to analyze the differential value of DCE-MRI hemodynamic parameters for high and low-grade gliomas.Pearson was used to analyze the correlation between hemodynamic parameters and molecular biological markers of cancer tissues.Results:The levels of DCE-MRI hemodynamic parameters Ktrans,Kep and Ve in the high-level group were higher than those in the low-level group(P<0.05).There was no significant difference in Vp level between the high-level group and the low-level group(P>0.05).The area under the ROC curve of combined detection of DCE-MRI hemodynamic parameters Ktrans,Kep and Ve in differentiating high and low grade glioma was 0.927,which was higher than that of single detection of 0.822,0.803 and 0.785,with sensitivity of 91.11%and specificity of 80.49%(P<0.05).The expression levels of VEGF mRNA and Ki-67 mRNA in the high-grade group were higher than those in the low-grade group(P<0.05).Pearson correlation analysis showed that the levels of Ktrans,Kep and Ve in DCE-MRI of glioma were positively correlated with the expression levels of VEGF mRNA and Ki-67 mRNA in cancer tissues(r=0.522,0.415,0.323,0.483,0.376,0.274,P<0.05).Conclusion:The levels of DCE-MRI hemodynamic parameters Ktrans,Kep and Ve in high-grade gliomas are higher than those in low-grade gliomas,and are positively correlated with the expression levels of VEGF and Ki-67,which can be used for clinical identification of high-grade and low
