补阳止眩汤联合常规西药治疗阳虚型高血压病眩晕临床研究

摘　　要：目的:观察补阳止眩汤联合常规西药治疗阳虚型高血压病眩晕的临床疗效。方法:选取128例阳虚型高血压病眩晕患者为研究对象,按随机数字表法分为对照组与联合组各64例。对照组采用苯磺酸氨氯地平片治疗,联合组采用苯磺酸氨氯地平片联合补阳止眩汤治疗。比较2组临床疗效、不良反应,比较2组治疗前后的动态血压,眩晕障碍量表(DHI)中躯体(DHIP)、情绪(DHIE)、功能(DHIF)各项评分,椎动脉收缩期峰值流速(VS)、舒张期流速(VD)。结果:治疗后,2组24 h平均收缩压(24 hSBP)、24 h平均舒张压(24 hDBP)均下降(P<0.05),且联合组低于对照组(P<0.05)。治疗后,2组DHI总分及DHIP、DHIE、DHIF评分均下降(P<0.05),且联合组低于对照组(P<0.05)。治疗后,2组椎动脉VS、VD值均升高(P<0.05),且联合组高于对照组(P<0.05)。联合组总有效率95.31%,高于对照组82.81%(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:补阳止眩汤联合常规西药治疗阳虚型高血压病眩晕能够有效降低血压,改善高血压病眩晕症状和椎动脉血流动力学,疗效显著且用药安全性良好。Objective:To observe the clinical effect of the therapy of Buyang Zhixuan Decoction combined with routine western medicine on dizziness due to hypertension of yang deficiency type.Methods:A total of 128 patients with dizziness due to hypertension of yang deficiency type were selected as the study subjects and divided into the control group and the combination group according to the random number table method,with 64 cases in each group.The control group was given Amlodipine Besilate Tablets for treatment,and the combination group was treated with Amlodipine Besilate Tablets combined with Buyang Zhixuan Decoction.The clinical effects,incidence of adverse reactions,dynamic blood pressure,scores of physical(DHIP),emotional(DHIE)and functional(DHIF)impacts in Dizziness Handicap Inventory(DHI),systolic peak velocity(VS)and diastolic velocity(VD)of vertebral artery were compared before and after treatment between the two groups.Results:After treatment,the average 24-hour systolic blood pressure(24 hSBP)and 24-hour diastolic blood pressure(24 hDBP)in the two groups were decreased(P<0.05),and the 24 hSBP and 24 hDBP in the combination group were lower than those in the control group(P<0.05).After treatment,the scores of DHIP,DHIE,DHIF in DHI and total score in both groups were decreased(P<0.05),and the above scores in the combination group were lower than those in the control group(P<0.05).After treatment,the values of VS and VD of vertebral artery in both groups were increased(P<0.05),and the above values in the combination group were higher than those in the control group(P<0.05).The total effective rate was 95.31%in the combination group,higher than that of 82.81%in the control group(P<0.05).There was no significant difference being found in the comparison of incidence of adverse reactions between the two groups(P>0.05).Conclusion:The therapy of Buyang Zhixuan Decoction combined with routine western medicine for dizziness due to hypertension of yang deficiency type can effectively decrease the blood pressure,and impro
