超声引导蝶腭神经节阻滞在功能性内窥镜鼻窦手术术后镇痛中的应用

摘　　要：目的探索超声引导蝶腭神经节阻滞在功能性内窥镜鼻窦手术术后镇痛中的应用优势与价值。方法研究以收治的功能性内窥镜鼻窦手术患者作为研究样本支撑本次研究,患者数量40例,时间节点截取:2021年7月至2023年6月,按照随机抽取盲信封方式将患者抽取为两组,患者均开展超声引导蝶腭神节阻滞术,对照组在蝶腭神经节附近注射0.9%的氯化钠2 ml,观察组在蝶腭神经节附近注射0.5%罗哌卡因2 ml,观察患者各项指标变化。结果观察组围手术期满意度分值相比于对照组更高,可见统计差(P<0.05)。观察组患者术后各时间段VAS评分(0.5 hVAS、6 hVAS、12 hVAS、24 hVAS、48 hVAS)相比对照组存在明显对比性,P<0.05。患者T0时间段平均动脉压、心率指标未见对比性(P>0.05);观察组患者各时间点(T_(1)、T_(2)、T_(3))指标相比对照组差值明显,P<0.05。观察组不良反应(5.00%)低于对照组(25.00%),P<0.05。结论功能性内窥镜鼻窦手术术后镇痛中超声引导蝶腭神经节阻滞应用价值显著,可以显著地降低患者疼痛感,保证血流动力学稳定,改善患者以后,降低不良反应,减少手术对患者产生的刺激性,促使患者尽早恢复健康,具有较高的应用价值,值得临床推广应用。Objective To explore the advantages and value of ultrasound-guided sphenopalatine ganglion block in postoperative analgesia after functional endoscopic sinus surgery.Methods The study was supported by the patients with functional endoscopic sinus surgery.The number of patients was 40,and the time node was intercepted:from July 2021 to June 2023,the patients were randomly selected into two groups,all of whom underwent ultrasound-guided sphenopalatine ganglion block.The control group injected 0.9%sodium chloride 2 ml near the sphenopalatine ganglion,and the observation group injected 0.5%ropivacaine 2 ml near the sphenopalatine ganglion to observe the patients.Results The perioperative satisfaction score of the observation group was higher than that of the control group,and the statistical difference was obvious(P<0.05).The VAS scores(0.5 hVAS,6 hVAS,12 hVAS,24 hVAS,48 hVAS)in the observation group were significantly different from those in the control group(P<0.05).The average arterial pressure and heart rate of patients at time T0 were not compared(P>0.05).Compared with the control group,the indexes in the observation group at each time point(T_(1),T_(2),T_(3))were significantly different(P<0.05).The adverse reactions ratio in the observation group(5.00%)was lower than the control group(25.00%),P<0.05.Conclusions Ultrasound-guided sphenopalatine ganglion block has significant application value in postoperative analgesia after functional endoscopic sinus surgery,which can significantly reduce patients'pain,ensure hemodynamic stability,improve patients,reduce adverse reactions,reduce the irritation caused by surgery,and promote patients to recover as soon as possible.It has high application value and is worthy of clinical application.
