腹腔镜与开腹阑尾残端荷包缝合治疗复杂性阑尾炎的临床疗效比较

摘　　要：目的探讨腹腔镜与开腹阑尾残端荷包缝合治疗复杂性阑尾炎的临床疗效。方法回顾性分析于2020年1月至2023年1月到本科室行阑尾切除术的100例急性复杂性阑尾炎患者作为研究对象,根据术中阑尾残端处理方式的不同将患者分为两组,腹腔镜组36例,沿盲肠壁的阑尾基底沟外侧行荷包缝合;开腹组64例,传统阑尾残端荷包缝合;比较两组术后肛门恢复排气时间、术后并发症发生情况、术后住院时间,住院费用等。结果两组患者的年龄、性别、术前体温、白细胞计数和C反应蛋白均无统计学差异(P>0.05),两组并发症发生率无统计学差异(P>0.05),均无阑尾残端漏发生。腹腔镜组与开腹组比较,术后肛门恢复排气时间快[(1.69±0.53)d vs.(2.05±0.52)d,P=0.002]、术后住院时间短[(5.36±1.85)d vs.(7.39±2.04)d,P=0.001],但是住院费用较高[(10981.05±1782.33)元vs.(8485.14±2283.34)元,P=0.001]。结论腹腔镜下阑尾残端沿基地沟荷包缝合与开腹手术治疗急性复杂性阑尾炎同样安全可靠,而且腹腔镜组术后恢复快,住院时间短。Objective To compare the clinical effects of purse sting suture stump embedding in the treatment of complicated appendicitis by laparoscopic and appendectomy.Methods A total of 100 cases of complicated appendicitis patients who underwent appendectomy in our department from January 2020 to January 2023 were selected as the research objects.According to the type of appendectomy,they were divided into laparoscopic group(n=36)and open group(n=64).The bowel function recovery time,postoperative complications,postoperative hospital stay and total operating coast were compared between the two groups.Results There were no significant difference in the baseline data of the two group,include age,gender,preoperative body temperature.Leukocyte count and CRP(P>0.05).There was no significant difference in the incidence of complications between the two groups,and no stump leakage occurred.Compared with the open group,the laparoscopic group had a faster postoperative anal recovery time[(1.69±0.53)d vs.(2.05±0.52)d,P=0.002]and a shorter postoperative hospitalization time[(5.36±1.85)d vs.(7.39±2.04)d,P=0.001],but higher hospitalization costs[(10981.05±1782.33)yuan vs.(8485.14±2283.34)yuan,P=0.001].Conclusions Laparoscopic and open appendiceal stump pouch suture are equally safe and reliable in the treatment of acute complicated appendicitis,but the laparoscopic group has faster postoperative recovery and shorter hospital stay.
