超声引导下改良塞丁格技术与传统PICC置管术的对比研究

摘　　要：目的对传统经外周静脉穿刺置入中心静脉导管(PICC)置管术和超声引导下改良塞丁格技术的效果进行对比研究。方法选取2019年2月至2021年1月期间住院治疗的120例患者,随机分为两组,每组各60例。对照组采用传统PICC置管术,观察组采用超声引导下改良塞丁格技术,观察两组的效果。结果对照组的一次置管成功率和导管异位率分别低于和高于观察组,差异有统计学意义(P<0.05)。置管次日两组的并发症无明显差异(P>0.05)。两组置管第5天并发症发生率比较,对照组高于观察组(P<0.05)。对照组置管第6天到拔管期间并发症发生率高于观察组,肢体舒适度低于观察组(P<0.05)。结论对比PICC传统置管技术和超声引导下改良塞丁格技术,后者的一次置管成功率更高,且不容易出现导管异位的情况,在置管后的并发症发生率相对更低,因此可以延长导管的使用时间,具有比较理想的置管效果,适合在临床置管患者中进行推广。Objective To compare the effects of traditional peripheral venipuncture central catheter(PICC)catheterization and ultrasound-guided modified Sedinger technique.Methods From February 2019 to January 2021,120 hospitalized patients were randomly divided into two groups of 60 cases.The control group received traditional PICC catheterization,while the observation group received improved Sedinger technique guided by ultrasound to observe the effects of both groups.Results The success rate of catheterization and catheter ectopic rate of control group were lower and higher than observation group,respectively,and the difference was statistically significant(P<0.05).There was no significant difference in complications between the two groups on the next day after catheter placement(P>0.05).The complications of the two groups on the 5th day of intubation were higher in the control group than in the observation group(P<0.05).The incidence of complications in the control group was higher than that in the observation group from the 6th day of catheterization to extubation,and the limb comfort was lower than the observation group(P<0.05).Conclusions Compared with the traditional PICC catheterization technique and the modified Sedinger technique under ultrasound guidance,the latter has a higher success rate of one-time catheterization,is less prone to catheter ectopic,and has a relatively higher incidence of complications after catheterization.Therefore,it can prolong the use time of the catheter and has an ideal catheterization effect,which is suitable for further promotion in clinical catheterization patients.
