苦参汤加减方联合肛门熏洗坐浴治疗痔术后肛缘水肿的效果

摘　　要：目的探讨苦参汤加减方联合肛门熏洗坐浴在术后肛缘水肿中的疗效。方法择取本院2022年2月至2023年2月收治的痔患者为研究对象,患者在接受手术治疗后并发肛缘水肿,共70例。以抽签法分组,随机分为两组,各有35例。对照组术后采用温盐水坐浴联合痔疮膏外涂,观察组术后采用苦参汤加减联合肛门熏洗坐浴治疗。比较两组患者水肿评分、治疗疗效、不良反应、生活质量、睡眠质量。结果两组治疗前水肿评分对比差异小(P>0.05);治疗后观察组水肿评分低于对照组(P<0.05)。观察组治疗总有效率97.14%(34/35),高于对照组总有效率71.43%(25/35)(P<0.05)。观察组不良反应发生率2.86%(1/35),对照组不良反应发生率5.71%(2/35),两组对比结果为P>0.05。治疗前两组患者世卫组织生活质量问卷(WHOqol)评分对比结果为P>0.05;治疗后观察组WHOqol评分高于对照组(P<0.05)。治疗前两组患者阿森斯睡眠质量评分(AIS)对比结果为P>0.05;治疗后观察组AIS评分低于对照组(P<0.05)。结论苦参汤加减方用于肛门熏洗坐浴可改善术后肛缘水肿严重程度,并且安全性高,有助于促进患者生活质量、睡眠质量提升。Objective To explore the therapeutic effect of modified Kushen decoction combined with anal fumigation and sitz bath on postoperative anal edema after hemorrhoid surgery.Methods A total of 70 hemorrhoid patients admitted to the hospital from February 2022 to February 2023 were selected,who developed anal edema after surgical treatment.Divide into two groups using random drawing method,with 35 cases in each group.The control group received warm saline sitz bath and hemorrhoid ointment after surgery,while the observation group received modified Kushen decoction combined with anal fumigation after surgery.Compare the edema,efficacy,side effects,quality of life,and sleep status between two groups of patients.Results The edema score,WHOqol score,and AIS score of two groups of patients before treatment(P>0.05);After treatment,the edema score and AIS score of the observation group were lower than those of the control group,while the WHOqol score was higher than that of the control group(P<0.05).The total effective rate of the observation group was as high as 97.14%,significantly higher than that of the control group(P<0.05).The adverse reaction rate in the observation group was as low as 2.86%,significantly lower than that in the control group(P<0.05).Conclusions The combination of Kushen decoction and anal fumigation and washing sitz bath has an ideal therapeutic effect and high safety in the treatment of postoperative anal edema after hemorrhoid surgery,which helps to improve the quality of life and sleep of patients.
