足浴联合低频脉冲治疗对糖尿病高危足病变患者足背动脉血流量的影响

摘　　要：目的 观察足浴联合低频脉冲治疗对糖尿病高危足病变患者足背动脉血流量的影响。方法 将120例糖尿病高危足患者按照随机数字表法分为2组,对照组60例予常规治疗,治疗组60例在对照组治疗基础上予足浴联合低频脉冲治疗。2组均治疗32周,比较2组治疗前后血清C反应蛋白(CRP)、肿瘤坏死因子α(TNF-α)、足背动脉血流和双下肢震动感觉阈值,并统计2组疗效。结果 治疗组总有效率96.67%(58/60),对照组总有效率81.66%(49/60),治疗组临床疗效优于对照组(P<0.05)。2组治疗后TNF-α、CRP水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后足背动脉血管管径、血管搏动指数、踝-肱指数均较本组治疗前增高(P<0.05),且治疗组治疗后均高于对照组(P<0.05),2组治疗后动脉收缩期血流速度均降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。2组治疗后双下肢震动感觉阈值均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。结论 糖尿病高危足患者予足浴联合低频脉冲治疗可以有效提高临床疗效,改善患者的震动感觉阈值和足背动脉血流,同时可以降低机体炎症因子水平,值得推广应用。Objective To investigate the effect of foot bath combined with low frequency pulse therapy on dorsal blood flow in patients with high-risk diabetic foot disease(DFD).Methods A total of 120 patients with high-risk DFD were randomly assigned into the treatment group(n=60)and the control group(n=60).All patients were managed by conventional therapy,and those in the treatment group were additionally treated with foot bath combined with low frequency pulse therapy.Treated for 32 weeks,the aim was to compare serum C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),ankle-brachial index(ABI)and vibration perception thresholds(VPT).The curative effect was assessed.Results The overall effective rate in the treatment group was better than that in the control group(96.67%[58/60]vs 81.66%[49/60],[P<0.05],respectively).After treatment,TNF-α,CRP in groups were significantly decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).After treatment,the diameter of dorsalis pedis artery,pulsatility index(PI),and ABI in the two groups were significantly higher than those in the group before treatment(P<0.05),and the systolic blood flow velocity and the VPT of foot were significantly lower(P<0.05),andwhich in the treatment group was superior to the control group(all P<0.05).Conclusion The combined application of foot bath combined with low frequency pulse therapy in patients with high-risk DFD can effectively improve the clinical efficacy,improve the ABI and VPT,and reduce inflammatory factors,and it is worthy of further popularization and application.
