滋阴降火汤联合针刺八髎穴对多囊卵巢综合征患者性激素及子宫内膜容受性的影响

摘　　要：目的观察滋阴降火汤联合针刺八髎穴对多囊卵巢综合征(PCOS)患者性激素及子宫内膜容受性的影响。方法将167例PCOS患者按照随机数字表法分为2组,对照组83例予常规西医治疗,治疗组84例在对照组治疗基础上予滋阴降火汤联合针刺八髎穴治疗,30天为1个疗程,2组均治疗3个疗程。比较2组治疗前后中医证候评分、性激素[卵泡生成激素(FSH)、黄体生成激素(LH)、雌二醇(E_(2))、睾酮(T)]和子宫内膜容受性相关指标[子宫内膜厚度、子宫内膜血流搏动指数(PI)、阻力指数(RI)、卵巢体积]水平,统计2组治疗期间的不良反应发生率。结果治疗组退出4例,对照组退出3例。2组治疗后中医证候各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后LH、E_(2)、T水平均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05),2组治疗后FSH水平均升高(P<0.05),且治疗组治疗后高于对照组(P<0.05)。2组治疗后子宫内膜厚度均较本组治疗前增加(P<0.05),且治疗组治疗后高于对照组(P<0.05),2组治疗后PI、RI和卵巢体积均降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论滋阴降火汤联合针刺八髎穴可有效改善PCOS患者的性激素水平和子宫内膜容受性,减轻临床症状,且安全性良好。Objective To explore the effect of Ziyin Jianghuo Decoction combined with acupuncture at Baliao point on sex hormones and endometrial receptivity in patients with polycystic ovary syndrome(PCOS).Methods A total of 167 PCOS patients were randomly assigned into the treatment group(n=84,4 cases dropped off)and the control group(n=83,3 cases dropped off).All patients were managed by conventional Western medicine,and those in the treatment group were additionally treated with Ziyin Jianghuo Decoction combined with acupuncture at Baliao points.A three-course treatment was performed in the both groups(30 d as a course),aiming to compare traditional chinese medicine(TCM)symptoms scores,sex hormone levels including follicle-stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_(2)),testosterone(T),endometrial receptivity-related indicators including endometrial thickness,endometrial blood flow pulsatility index(PI),resistance index(RI)and ovarian volume between the two groups.The treatment-emergent adverse events(TEAEs)were assessed.Results After treatment,the TCM symptom scores,LH,E_(2),T levels of the two groups were significantly lower(P<0.05),and FSH levels were significantly higher(P<0.05),and the increase/decrease trend of the treatment group was better than that of the control group,with a statistically significant difference(P<0.05).The endometrial thickness of the two groups increased significantly(P<0.05),PI,RI and ovarian volume decreased significantly(P<0.05),and the increase/decrease trend of the treatment group was better than that of the control group,with a statistically significant difference(P<0.05).There was no statistical significant difference in TEAEs between groups(P>0.05).Conclusion Ziyin Jianghuo Decoction combined with acupuncture at Baliao point can effectively improve the sex hormone level and endometrial receptivity of PCOS patients,and the clinical symptoms of patients can be significantly reduced with good safety,which can provide guidance for the follow-up treatment of PCOS.
