小续命汤足浴联合穴位按摩对急性缺血性脑卒中恢复期下肢运动功能障碍的多中心随机对照研究

摘　　要：目的:探讨小续命汤足浴联合穴位按摩对急性缺血性脑卒中恢复期下肢运动功能障碍患者的恢复疗效及护理效果。方法:将90例急性缺血性脑卒中恢复期患者随机分为对照组47例和观察组43例。对照组采用常规康复护理,观察组在对照组基础上予小续命汤足浴联合穴位按摩护理。治疗前和治疗4周后,采用Berg平衡量表、Fugl-Meyer评定量表(FMA)下肢运动功能评定量表、Brunnstrom 6级评定法、改良Barthel指数(MBI)、SF-36量表评定患者治疗前后平衡能力、下肢肢体运动功能、患侧下肢运动功能级别、日常生活活动能力、生存质量。结果:治疗后两组患者的Berg平衡量表评分都有提高,且观察组提高的幅度更大(P<0.01);治疗后观察组FMA-LE评分显著高于对照组(P<0.01);两组患者患肢运动功能级别较干预前明显改善(P<0.05),且观察组改善显著优于对照组(P<0.01);治疗后观察组改良Barthel指数评分显著高于对照组(P<0.01);干预后两组患者健康调查简表SF-36各维度评分均较治疗前明显改善,且观察组患者健康调查简表SF-36各维度评分均显著高于对照组(P<0.01)。结论:小续命汤足浴联合穴位按摩可明显提升急性缺血性脑卒中恢复期患者下肢肢体功能的恢复,提高患者的生存质量,值得临床推广。Objective:To explore the recovery efficacy and nursing effect of Xiaoxuming Decoction foot bath combined with acupoint massage on patients with lower limb motor dysfunction during the recovery period of acute ischemic stroke.Method:Ninety patients recovering from acute ischemic stroke were randomly divided into 47 cases in the control group and 43 cases in the observation group.The control group received routine rehabilitation care,while the observation group received Xiaoxuming Decoction foot bath combined with acupoint massage care on the basis of the control group.Before and after 4 weeks of treatment,the Berg Balance Scale,Fugl-Meyer Rating Scale(FMA),Brunnstrom Level 6 Rating Scale,Modified Barthel Index(MBI),and SF-36 Scale were used to assess the patient's balance ability,lower limb motor function,affected lower limb motor function level,daily living activity,and quality of life before and after treatment.Result:After treatment,the Berg balance scale scores of both groups of patients improved,and the observation group showed a greater increase(P<0.01);After treatment,the FMA-LE score in the observation group was significantly higher than that in the control group(P<0.01);The motor function level of the affected limbs in both groups of patients was significantly improved compared to before intervention(P<0.05),and the improvement in the observation group was significantly better than that in the control group(P<0.01);After treatment,the improved Barthel index score in the observation group was significantly higher than that in the control group(P<0.01);After intervention,the scores of each dimension in the SF-36 health survey summary table for both groups of patients were significantly improved compared to before treatment,and the scores of each dimension in the SF-36 health survey summary table for the observation group were significantly higher than those in the control group(P<0.01).Conclusion:The combination of Xiaoxuming Decoction foot bath and acupoint massage can significantly improve the recovery of
