魏开建教授治疗桥本甲状腺炎临证经验

摘　　要：桥本甲状腺炎是一种自身免疫性甲状腺疾病,临床上多对症治疗。文章总结魏开建教授对桥本甲状腺炎病因病机的认识及临证经验。桥本甲状腺炎与遗传、情绪、饮食密切相关,气、痰、瘀病理产物结于颈前而发病。魏开建教授认为在桥本甲状腺炎诊治过程中应先辨病,再辨证,将桥本甲状腺炎分为5个证型,分别是肝郁气滞证、气滞痰凝证、痰瘀互结证、气阴两虚证、脾肾阳虚证,运用疏肝理气、理气化痰、活血化瘀、补益肝肾、化痰散结等中药辨证论治。魏开建教授强调桥本甲状腺炎起病多因情志不舒,使气滞、痰凝于颈前而发病,故临证中擅从肝脾论治,收到较好的效果。Hashimoto's thyroiditis is a kind of autoimmune thyroid disease,many clinical symptomatic treatment.This paper summarizes Professor Wei Kaijian's understanding of the etiology and pathogenesis of Hashimoto's thyroiditis and his clinical experience.Hashimoto's thyroiditis is closely related to genetics,mood and diet,and the pathological products of Qi,sputum and blood stasis occur in front of the neck.Professor Wei Kaijian believed that in the process of diagnosis and treatment of Hashimoto's thyroiditis,the disease should be first distinguished and then dialectical.Hashimoto's thyroiditis was divided into five syndromic types,namely syndrome of liver depression and Qi stagnation,syndrome of Qi stagnation and phlegm stagnation,syndrome of phlegm and blood stasis,syndrome of Qi and Yin deficiency,and syndrome of spleen and kidney Yang deficiency.The dialectical treatment of Hashimoto's thyroiditis was carried out with TCM such as soothing the liver and regulating Qi,regulating Qi and phlegm,promoting blood circulation and blood stasis,tonifying the liver and kidney,and resolving phlegm and dispersing.Professor Wei emphasized that the onset of Hashimoto's thyroiditis is often caused by emotional discomfort,so that Qi stagnation and spittoon are congeated in front of the neck.Therefore,the clinical syndrome is good at treating from the liver and spleen,and good results are received.
