摘　　要：目的对2012—2021年北京市海淀区HIV筛查阳性样本的确证试验中HIV抗体不确定结果进行分析。方法对HIV筛查实验室上送的8454例次筛查阳性样本进行免疫印迹试验(western blot,WB),分析不确定样本的带型特点和随访情况,并用SPSS 24.0软件进行统计分析。结果在8454例次筛查阳性样本中,HIV抗体不确定样本1895例次,占比22.4%,主要来自医疗机构(50.5%),其次是血液中心(47.8%)。WB条带出现率最高是gag蛋白中的p24(78.7%)和env蛋白中的gp160(17.7%)。在完成随访的211例样本中,其中35例(16.6%)转为阳性,79例(37.4%)转为阴性。18~44岁年龄组的男性,尤其是有男男性行为的转阳率较高。条带越多转阳率越高。结论北京市海淀区HIV抗体不确定结果存在较高假阳性。尽管病例随访率低,但是仍发现一些急性期阳转病例。应加大随访力度,做到早发现早诊断早治疗。Objective To analyze the indeterminate results of HIV antibody in the confirmatory test among HIV screening positive samples of Haidian district of Beijing from 2012 to 2021.Methods Western blot(WB)tests were performed for 8454 HIV screening positive samples from the HIV screening laboratories in Haidian district.The characteristics of WB band patterns of indeterminate samples and the results of follow-up were analyzed.SPSS 24.0 was used for statistical analysis.Results Among the 8454 HIV screening positive samples,1895 were indeterminate,accounting for 22.4%.The samples were mainly from medical institutions(50.5%),followed by blood centers(47.8%).The most frequently detected WB bands were p24 of gag protein(78.7%)and gp160 of env protein(17.7%).For 211 cases with completed followed up,the results turned into positive in 35 cases(16.6%)and negative in 79 cases(37.4%).The positive rate was higher among males in the age group of 18-44 years old,especially for those who had sex with men.The more bands were detected,the higher positive rate was in the follow-up.Conclusions The false positives in the indeterminate results of HIV antibody test in Haidian district of Beijing was rather high.Although the rate of follow-up was low,still some cases of positive conversion in acute stage were found.Follow-up should be strengthened for early detection,early diagnosis and early treatment.
