摘 要:目的系统探讨流感疫苗对新冠病毒(severe acute respiratory syndrome coronavirus 2,SARS-CoV-2)感染患者临床结局严重程度的影响。方法通过计算机检索中英文数据库,检索接种流感疫苗和未接种流感疫苗对SARS-CoV-2感染影响的文献,进行质量评价后采用Revman 5.3分析接种流感疫苗对SARS-CoV-2感染患者感染率、住院率、肺炎发生率、重症率、机械通气率、住院时间、死亡率的影响。结果共纳入13篇文献,总计纳入样本66281例,其中接种流感疫苗25447例,未接种流感疫苗40834例。合并Meta分析显示,接种流感疫苗人群SARS-CoV-2感染重症率明显降低(OR=0.63,95%CI:0.41~0.98,P=0.04)。结论接种流感疫苗可降低SARS-CoV-2感染患者的重症率,其对SARS-CoV-2感染患者感染率、住院率、住院时间、肺部炎症发生率、死亡率等方面未显示出改善作用,其对SARS-CoV-2感染患者机械通气率的影响尚需要进一步研究。Objective To systematically explore the impact of influenza vaccine on the severity of clinical outcomes in severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infected patients.Methods The Chinese and English databases were searched by computer for literatures on the impact of influenza vaccination and non-vaccination on the prognosis of SARS-CoV-2 infected patients.After the quality evaluation,Revman 5.3 was used to analyze the impacts of influenza vaccination on the infection rate,hospitalization rate,incidence rate of pneumonia,incidence rate of severe cases,rate of mechanical ventilation,hospitalization time as well as mortality among SARS-CoV-2 infected patients.Results A total of 13 articles were enrolled,including 66281 subjects.Among the subjects,25447 received influenza vaccine and 40834 did not.Combined meta-analysis showed that the incidence rate of severity decreased significantly among SARS-CoV-2 infected patients who received influenza vaccine(OR=0.63,95%CI=0.41-0.98,P=0.04).Conclusions Influenza vaccination can reduce the severity rate of SARS-CoV-2 infected patients.There was no clear improvement in the infection rate,hospitalization rate,hospitalization time,incidence of pulmonary inflammation and mortality of SARS-CoV-2 infected patients.The rate of mechanical ventilation among SARS-CoV-2 infected patients needs further study.
