摘　　要：目的 分析急性胰腺炎严重程度床边指数(BISAP)评分、白细胞介素-6(IL-6)、降钙素原(PCT)与重症急性胰腺炎(SAP)预后的相关性。方法 选取2020年1月-2023年9月我院收治的86例SAP患者,根据预后情况分为死亡组(27例)、生存组(59例)。所有患者入院后均予以胃肠减压、禁食、抗感染等对症治疗及预防并发症治疗,并检测患者入院时的BISAP评分、血清IL-6、PCT水平。比较生存组、死亡组的BISAP评分、IL-6及PCT水平,分析SAP患者预后不良的危险因素,采用Pearson直线相关分析IL-6、PCT水平及BISAP评分与SAP预后的相关性。结果 死亡组的BISAP评分、血清IL-6、PCT水平高于生存组(P<0.05)。经Logistic回归分析显示,BISAP评分及血清IL-6、PCT水平较高是SAP患者预后不良的危险因素(OR>1,P<0.05)。Pearson直线相关分析显示,BISAP评分及血清IL-6、PCT水平与SAP患者预后不良呈正相关(r>0,P<0.05)。结论 SAP患者的BISAP评分及血清IL-6、PCT水平较高与预后不良密切相关。Objective To analyze the correlation between the severity bedside index(BISAP)score,interleukin-6(IL-6),procalcitonin(PCT),and prognosis in severe acute pancreatitis(SAP).Method 86 SAP patients admitted to our hospital from January 2020 to September 2023 were selected and divided into a death group(27 cases)and a survival group(59 cases)based on their prognosis.After admission,all patients were treated with targeted treatments such as gastrointestinal decompression,fasting,anti infection,and prevention of complications.The BISAP score,serum IL-6,and PCT levels of the patients were measured at admission.Compare the BISAP scores,IL-6,and PCT levels between the survival group and the death group,analyze the risk factors for poor prognosis in SAP patients,and use Pearson linear correlation to analyze the correlation between IL-6,PCT levels,and BISAP scores and SAP prognosis.Result The BISAP score,serum IL-6,and PCT levels in the death group were higher than those in the survival group(P<0.05).Logistic regression analysis showed that BISAP score and high levels of serum IL-6 and PCT were risk factors for poor prognosis in SAP patients(OR>1,P<0.05).Pearson linear correlation analysis showed that the BISAP score and serum IL-6,PCT levels were posi-tively correlated with poor prognosis in SAP patients(r>0,P<0.05).Conclusion The BISAP score and high levels of serum IL-6 and PCT in SAP patients are closely related to poor progno-sis.
