摘  要:目的探讨微血流成像技术(MFI)在牵拉性视网膜脱离诊断中的应用价值。方法回顾性病例研究。选择增殖型糖尿病视网膜病变患者125例,分别采用彩色多普勒血流成像(CDFI)和超声MFI观察患眼,对比病理膜血流信号的相关情况。利用受试者工作特征(ROC)曲线分析两种方式的检测灵敏度、特异度,并找到合适的临界点。结果125例糖尿病视网膜病变患者中男性73例,占58.4%;女性52例,占41.6%。其中非增殖型糖尿病视网膜病变94只眼,占37.6%;增殖型糖尿病视网膜病变156只眼,占62.4%;增殖型糖尿病视网膜病变合并牵拉性视网膜脱离共87只眼,占55.77%。CDFI的ROC曲线对应的曲线下面积(AUC)为0.856(95%CI:0.795~0.917),对应的最佳临界值为0.400(灵敏度为71.3%,特异度为100%);MFI的ROC曲线对应的AUC为0.978(95%CI:0.95~0.99),对应的最佳临界值为0.716(灵敏度为100%,特异度为95.7%)。MFI的诊断灵敏度较CDFI更高,差异有统计学意义(P<0.05)。结论应用彩色多普勒超声诊断增殖型糖尿病视网膜病变合并牵拉性视网膜脱离是一种便捷、重复性好的方法,利用MFI可提高诊断的准确度。Objective To evaluate the value of microvisual flow imaging(MFI)in the diagnosis of traction retinal detachment.Methods Retrospective case study.A total of 125 patients with proliferative diabetic retinopathy who were hospitalized.Color Doppler flow imaging(CDFI)and MFI were used to observe the affected eyes,and the relevant conditions of blood flow signals in pathological membrane were compared.Receiver operating characteristic(ROC)curve was used to analyze the sensitivity and specificity of the two methods,and the appropriate critical point was found.Results Among 125 patients with diabetic retinopathy,73 were male,accounting for 58.4%.52 cases(41.6%)were female.There were 94 eyes with non-proliferative diabetic retinopathy(37.6%).Proliferative diabetic retinopathy was found in 156 eyes(62.4%).Proliferative diabetic retinopathy combined with traction retinal detachment was observed in 87 eyes(55.77%).The area under the curve(AUC)of ROC corresponding to CDFI was 0.856(95%CI:0.795-0.917),and the corresponding optimal cut-off value was 0.400(sensitivity 0.713,specificity 1).The AUC of ROC curve corresponding to MFI was 0.978(95%CI:0.95-0.99),and the corresponding best cut-off value was 0.716(sensitivity was 1,specificity was 0.957).The diagnostic sensitivity of ultrasonic MFI was higher than that of CDFI,and the difference was statistically significant(P<0.05).Conclusions Color Doppler ultrasonography is an economical,convenient and reproducible method for the diagnosis of proliferative diabetic retinopathy combined with traction retinal detachment,and MFI can improve the diagnostic accuracy.
