摘  要:目的 探讨超声影像组学在甲状腺皱缩结节(MTN)与甲状腺乳头状癌(PTC)鉴别诊断中的价值。方法 纳入213例病理诊断为MTN或PTC的患者,并按7∶3随机分为训练集(37例MTN和112例PTC)与验证集(21例MTN和43例PTC)。从每个病例超声图像中提取了1 239个影像组学特征,经降维筛选后建立超声影像组学评分模型。通过校准曲线、受试者工作特征(ROC)曲线评价模型的预测效能,使用决策曲线分析(DCA)评估其临床应用价值。结果 选择16个特征构建超声影像组学评分模型。校准曲线显示模型的一致性较好,训练集与验证集的曲线下面积(AUC)分别为0.878(95%CI:0.816~0.937)、0.854(95%CI:0.752~0.955)。DCA表明其具有临床价值。结论 超声影像组学是鉴别MTN与PTC的一个有潜能的工具。Objective To explore the value of ultrasound-based radiomics in the differential diagnosis of mummified thyroid nodule(MTN)and papillary thyroid carcinoma(PTC).Methods A total of 213 patients with pathological diagnosis of either MTN or PTC were enrolled in a training cohort(37 MTN cases and 112 PTC cases)and validation cohort(21 MTN cases and 43 PTC cases)using random sampling with a 7∶3 ratio.Overall,1239 radiomics features were extracted from ultrasound images for each case.After dimensionality reduction and screening,an ultrasound-based radiomics score model was established.The predictive efficacy of the model was assessed using the calibration curve,receiver operating characteristic(ROC)curve,and clinical usefulness was determined using decision curve analysis(DCA).Results Sixteen features from ultrasound images were used to build the ultrasound-based radiomics score model.The calibration curves demonstrated good calibration of the model.The area under the curve(AUC)of the model was 0.878(95%CI:0.816~0.937)and 0.854(95%CI:0.752~0.955)in the training and validation cohorts,respectively.DCA showed that the model had clinical utility.Conclusions Ultrasound-based radiomics is apromising tool for distinguishing between MTN and PTC.
