摘 要:目的 探讨超声在先天性结核病(CTB)中的应用价值。方法 回顾性分析儿童结核科就诊的CTB患儿,收集患者的一般临床资料及影像学检查结果。对比超声与CT对CTB的诊断差异。结果 本研究共纳入13例CTB患儿,单纯肺结核2例,肺结核合并肺外结核11例。超声及CT均检出肺周围型病灶13例;CT检出中央型病灶11例、肺空洞1例,肺门及纵隔淋巴结肿大8例,以上病灶超声均无法探及;对于胸膜增厚、胸腔积液、腹腔结核、浅表淋巴结结核、肝脾结核,超声与CT的检出率一致。结论 超声对于CTB肺周围型病灶、腹腔器官病灶及淋巴结病灶的检出率与CT是相当的。超声检查无辐射、可重复操作,可以作为CTB患儿首选筛查方法及治疗后的疗效评估。Objective To investigate the application value of ultrasound in congenital tuberculosis(CTB).Methods Retrospective analysis was performed on CTB patients admitted to the department of children's tuberculosis.General clinical data and imaging results of the patients were collected.The difference between ultrasound and computed tomography(CT)in the diagnosis of CTB was compared.Results A total of 13 children with CTB were included in this study,including 2 with simple pulmonary tuberculosis and 11 with pulmonary tuberculosis combined with extrapulmonary tuberculosis.Ultrasound and CT both detected peripheral lesions in 13 patients.CT detected central lesions in 11 patients,pulmonary cavitation in 1 patient,hilar and mediastinal lymph node enlargement in 8 patients,all of which missed by ultrasound.The detection rates of pleural thickening,pleural effusion,abdominal tuberculosis,superficial lymph node tuberculosis,liver tuberculosis and spleen tuberculosis by ultrasound and CT were consistent.Conclusions The detection rate of peripulmonary lesions,abdominal organ lesions,and lymph node lesions in CTB patients by ultrasound is comparable to that of CT.Ultrasound is radiation-free and repeatable,and can be used as the preferred screening method and the efficacy evaluation method of treatment for children with CTB.
