摘　　要：目的基于倾向指数匹配法分析日间手术腹腔镜胆囊切除术(LC)的实施效果。方法采用回顾性研究,选择江苏省苏北人民医院2019年7月1日—2022年6月30日行LC日间手术和非日间手术患者,比较两组的住院费用、费用构成和术后愈合情况。结果匹配后混杂因素在两组患者之间达到较好的均衡(p>0.05);日间组次均总费用中位数为12866.95元,非日间组为14586.43元(p<0.001),日间组除治疗费和耗材费外,其他费用构成均低于非日间组(p<0.05);日间组Ⅱ类切口甲级愈合相对低于非日间组(p<0.001),出院30天再住院计划两组无统计学意义(p﹥0.05)。结论日间手术对降低患者的住院费用有较好的促进作用,同时日间手术患者院外并发症的发生、健康宣教和应急预案应予以关注。Objective To analyze the implementation effect of day surgery laparoscopic cholecystectomy(LC)based on propensity score matching method.Methods A retrospective study was conducted to select patients who underwent LC day surgery and non-day surgery in Northern Jiangsu People's Hospital from July 1,2019 to June 30,2022.The hospitalization costs,cost composition and postoperative healing of the two groups were compared.Results After matching,the confounding factors were well balanced between the two groups(p>0.05).The median total cost per visit was 12866.95 yuan in the day group and 14586.43 yuan in the non-day group(p<0.001).Except for the cost of treatment and consumables,the composition of other expenses in the day group was lower than that in the non-day group(p<0.05).Grade A healing of typeⅡincision in the day group was relatively lower than that in the non-day group(p<0.001).There was no significant difference in the 30-day readmission plan between the two groups(p>0.05).Conclusion Day surgery has a good promoting effect on reducing the hospitalization cost of patients.At the same time,the occurrence of complications outside the hospital,health education and emergency plan of day surgery patients should be paid attention to.
