以六磨汤为基础治疗便秘型肠易激综合征的Meta分析

摘　　要：目的:系统评价以六磨汤为基础治疗便秘型肠易激综合征(Irritable Bowel Syndrome with Predominant Constipation,IBS-C)的临床疗效和安全性。方法:利用计算机检索中国生物医学文献数据库、中国知网、万方、维普、PubMed、Embase、The Cochrane Library中以六磨汤为基础治疗IBS-C的随机对照试验的相关文献,检索时间为自各数据库建库至2022年9月18日。由两名研究者独立选择文献、提取有效数据,对文献进行质量评价。Meta分析由Stata15软件完成。结果:最终纳入10篇文献,异质性检验得到I2=0.0%,P=0.580,Meta分析采用固定效应模型,得到相对危险度(RR)=1.24,95%置信区间(CI)(1.17,1.31),P<0.01,差异具有统计学意义,以六磨汤为基础的治疗组总有效率约为对照组的1.24倍。再以单纯六磨汤加味治疗与六磨汤加味联合其他治疗(西药或外治等)为分组依据,进行亚组分析,发现单纯六磨汤加味治疗组RR=1.25,95%CI(1.15,1.36),P<0.01,差异具有统计学意义,六磨汤加味联合其他治疗组RR=1.22,95%CI(1.13,1.33),P<0.01,差异具有统计学意义。RR值提示单纯六磨汤加味治疗组总有效率约为对照组的1.25倍,而六磨汤加味联合其他治疗组总有效率约为对照组的1.22倍。结论:基于六磨汤治疗IBS-C的效果显著,且单纯六磨汤治疗效果较六磨汤联合其他手段治疗效果更佳。但该结论仍需要更多高质量、多中心、大样本的随机对照试验进一步验证。Objective:To evaluate the clinical efficacy and safety of Liumo decoction(六磨汤)in the treatment of irritable bowel syndrome with predominant constipation(IBS-C).Methods:The randomized controlled studies(RCTs)on Liumo decoction in the treatment of IBS-C in China Biology Medicine disc,CNKI,Wanfang,VIP,PubMed,Embase and The Cochrane Library were searched by computer.The retrieval period was from the establishment of each database to September 18,2022.Two researchers independently selected literatures,extracted valid data,and evaluated the quality of literature.Meta-analysis was performed by Stata15 software.Results:A total of 10 articles were finally included,and the heterogeneity test showed I2=0.0%,P=0.580.A fixed effect model was used for a Meta-analysis,and the difference was statistically significant,with RR=1.24,95%CI(1.17,1.31),P<0.01.The total effective rate in the treatment group based on Liumo decoction was about 1.24 times that in the control group.Subgroup analysis was carried out based on Modified Liumo decoction alone and Modified Liumo decoction combined with other treatments(western medicine or external treatment,etc),and it was found that RR=1.25,95%CI(1.15,1.36),P<0.01 in the simple Liumo decoction treatment group,the difference was of statistical significance,wiht RR=1.22,95%CI(1.13,1.33),P<0.01 in Modified Liumo decoction combined with other treatments group.RR value indicated that the total effective rate of Modified Liumo decoction alone was about 1.25 times that in the control group,while that of Modified Liumo decoction combined with other treatments was about 1.22 times that in the control group.Conclusion:Liumo decoction is effective in the treatment of IBS-C,and the therapeutic effect of Liumo decoction alone is better than that of Liumo decoction combined with other treatments.But more high-quality,multicenter,large-sample randomized controlled trials are needed for further verification.
