健脾化湿法治疗2型糖尿病合并肥胖的研究进展

摘　　要：近年来,2型糖尿病合并肥胖患病率显著增加。流行病学研究表明,肥胖是2型糖尿病的独立危险因素,二者具有共同的病理基础和发病机制。2型糖尿病患者往往伴有超重和肥胖,导致胰岛素抵抗和高胰岛素血症、胰岛素分泌相对或绝对不足、高血脂等代谢紊乱,从而进一步促进了肥胖的发生发展。2型糖尿病合并肥胖作为一种新的疾病状态,其发病机制尚未明确,目前主要认为与胰岛素抵抗、肥胖、高血脂、脂代谢紊乱等多种因素相关。中医对该病的认识历史悠久,在长期临床实践中不断完善,积累了丰富的经验。其中,从健脾化湿角度论治2型糖尿病合并肥胖是目前临床研究的热点之一。2型糖尿病和肥胖两者之间相互独立又紧密相关,其病因病机多为脾气亏虚,痰湿内盛,治以健脾化湿法为主。目前,从健脾化湿角度治疗2型糖尿病合并肥胖的理论在不断完善,但仍缺乏系统的论述,导致对其临床疗效的不够深入、研究成果较少,有待进一步探索。文章基于对本病的认识,从中医病因病机及多角度治疗的角度探讨健脾化湿法治疗2型糖尿病合并肥胖的研究进展,并总结其不足之处,以期为糖尿病合并肥胖相关疾病的防治提供更多的参考方向和研究思路。In recent years,the prevalence of type 2 diabetes combined with obesity has increased significantly.Epidemiological studies have shown that obesity is an independent risk factor for type 2 diabetes,and the two have a common pathological basis and pathogenesis.Type 2 diabetes patients are often accompanied by overweight and obesity,resulting in insulin resistance and hyperinsulinemia,relative or absolute insufficiency of insulin secretion,hyperlipidemia and other metabolic disorders,which further promote the occurrence and development of obesity.As a new disease state,the pathogenesis of type 2 diabetes combined with obesity is not yet clear,and it is currently mainly believed to be related to a variety of factors such as insulin resistance,obesity,hyperlipidemia and disorders of lipid metabolism.Traditional Chinese medicine has a long history of understanding the disease,and has accumulated rich experience in long-term clinical practice.Among them,the treatment of type 2 diabetes combined with obesity from the perspective of invigorating spleen and relieving dampness is one of the current clinical research hotspots.Type 2 diabetes and obesity are independent and closely related to each other.The etiology and pathogenesis of type 2 diabetes are mostly deficiency of spleen,excessive phlegm and dampness,and the main treatment is strengthening spleen and relieving dampness.The theory of treating type 2 diabetes combined with obesity form the perspective of invigorating spleen and relieving dampness is constantly improving,but there is still a lack of systematic discussion,resulting in a lack of in-depth research on its clinical efficacy and fewer research results,which needs to be further explored.Based on the previous understanding of the disease,this paper discusses the research progress of invigorating spleen and relieving dampness in the treatment of type 2 diabetes mellitus combined with obesity from the perspectives of etiology,pathogenesis and multiple perspectives treatment in traditional Chinese medicine,and
