中药联合体外冲击波治疗肱骨外上髁炎的临床研究

摘　　要：目的:探讨中药复方制剂联合体外冲击波治疗肱骨外上髁炎的效果。方法:选取2018年6月―2020年11月在江苏省靖江市中医院骨伤科就诊的肱骨外上髁炎患者46例作为研究对象,采用随机数字表法将所有研究对象分为对照组和观察组,每组23例,对照组采用单纯体外冲击波治疗,观察组将自拟中药复方与超声耦合剂调成凝胶状,配合体外冲击波治疗并在患处外敷,疗程为1个月,比较治疗前后两组患者的视觉模拟评分法(Visual Analogue Scale,VAS)评分和Mayo肘关节功能评分。结果:治疗后两组患者的VAS评分较治疗前均明显降低(P<0.01),且观察组低于对照组(P<0.05);治疗后两组患者的Mayo肘关节功能评分较治疗前均明显提高(P<0.01),且观察组高于对照组,差异有统计学意义(P<0.05),表明观察组和对照组均能显著降低患者VAS评分,提高患者肘关节功能,且观察组较对照组疗效更优。两组患者在治疗周期内均未出现不良反应。结论:研究证明,以耦合剂为媒介,让中药复方制剂参与到肱骨外上髁炎治疗的全过程,是一项剂型和治疗方式的探索和创新,该方案联合体外冲击波治疗肱骨外上髁炎临床疗效确切,无不良反应,值得临床推广。Objective:To investigate the effect of Chinese herbal compound preparation combined with extracorporeal shock wave in the treatment of external humeral epicondylitis.Methods:A total of 46 patients with external humeral epicondylitis treated in the Orthopedics Department of Jingjiang Hospital of Traditional Chinese Medicine from June 2018 to November 2020 were selected as the study subjects.All the subjects were divided into the control group and the observation group by random number table method,with 23 cases in each group.The control group was treated by extracorporeal shock wave therapy alone,and the observation group was treated with Chinese herbal compound and ultrasonic coupling agent,which was prepared into a gel shape,combined with extracorporeal shock wave therapy and external application on the affected area for 1 month.VAS scores and Mayo elbow function scores in the two groups were compared before and after treatment.Results:After treatment,the VAS score in two groups was significantly decreased compared with that before treatment(P<0.01),and the score in the observation group was lower than that in the control group(P<0.05).After treatment,the Mayo elbow function score in two groups were significantly higher than those before treatment(P<0.01),and the score in the observation group was higher than the control group(P<0.05),the difference was statistically significant,indicating that both the observation group and the control group could significantly reduce the VAS score and improve the elbow function of patients,and the observation group was better than the control group.There were no adverse reactions in the two groups during the treatment cycle.Conclusion:Studies have proved that it is an exploration and innovation of dosage forms and treatment methods to involve Chinese herbal compound preparation in the whole process of treating external humeral epicondylitis with coupling agent as the medium.This scheme combined with extracorporeal shock wave has definite clinical efficacy in the treatment of e
