冉雪梦“引火归元”法在妇科疾病的经验举隅

摘　　要：引火归元一词,起源于《黄帝内经》,首见于《景岳全书》,属于中医学“热因热用”的反治法之一。其本质是运用温药将浮游于上之肾火向下引回到命门之中,使肾中水火相济,从而平衡人体阴阳,以达到阴平阳秘的效果。肾藏精,为天癸之源、冲任之本,肾精是人体生命活动的原动力,主宰女性生长发育、生殖衰老。肾为水火之脏、五脏阴阳之本,内藏真阴真阳,肾中阴阳平衡协调,才能维持机体正常的生理功能。肾与女子月经、带下、妊娠密切相关,肾的功能失调在妇科疾病的发病原因中占有重要地位。脾主运化,为后天之本,气血生化之源,是形成相火的物质基础。脾肾先后天相互依赖,相互资助。从病理上讲,脾肾互为因果,互相影响。女子具有经、带、胎、产、乳独特的生理特点,以血为用,因此引火归元法应用于妇科疾病的诊疗时有其独特的选方用药原则。文章总结冉雪梦主任医师运用引火归元法治疗妇科常见疾病的临证经验,凡临床辨证属阴虚火旺,阳虚火浮,阴盛格阳,土不伏火者,皆可根据引火归元法巧妙运用,常取得满意的疗效。文章选取3个案例,探讨引火归元法在妇科的具体应用,为临床诊疗提供思路。The method of conducting fire back to the origin originates from Huangdi Neijing(《黄帝内经》)and is first seen in Jingyue Quanshu(《景岳全书》).It is one of the reversed-treatment method of treating false-heat syndrome by heat methods in TCM.The essence is to use warm medicine to make the floating fire return to the gate of vitality,so that the kidney is in the harmony of water and fire,so as to balance Yin(阴)and Yang(阳)and achieve a relative equilibrium of Yin and Yang.Kidney essence is the source of Tiangui(天癸)and the foundation of Chongren(冲任)meridians,and it plays a significant role in the growth,development,reproduction and apolexis of female.The kidney is the organ of water and fire,and the foundation of Yin and Yang in the five organs.The balance and coordination of Yin and Yang are necessary to maintain normal physiological functions.Kidney is closely related to women’s menstruation,leucorrhea and pregnancy,and its dysfunction is a common cause of gynecological diseases.The spleen is the foundation of acquired constitution,the formation of the material basis of fire.The spleen and kidney rely on and promote each other.Because of the unique physiological characteristics of women,the method of conducting fire back to the origin has its unique principle of prescription selection when applied to the diagnosis and treatment of gynecological diseases.This article summarizes RAN Xuemeng’s experience.The method of conducting fire back to the origin can be flexibly applied in the clinical syndroms of Yin deficiency and fire flourishing,Yang deficiency and fire floating,Yin flourishing overwhelming Yang,and floating of deficient fire,and it often obtains satisfactory curative effects.This article selects 3 cases to discuss the specific application of conducting fire back to the origin treatment of gynecological diseases,and provides ideas for clinical diagnosis and treatment.
