凉血化斑汤加味治疗血热内蕴证寻常型银屑病临床研究

摘　　要：目的:观察凉血化斑汤加味治疗血热内蕴证寻常型银屑病的临床疗效。方法:收集寻常型银屑病患者88例,按随机数字表法分为对照组和治疗组各44例。对照组给予阿维A胶囊治疗,治疗组在对照组基础上加用凉血化斑汤加味治疗。比较2组临床疗效,以及治疗前后银屑病皮损面积和严重程度指数(PASI)、皮肤病生活质量指数(DLQI)、血热内蕴证证候评分以及血液流变学指标变化。结果:治疗后,治疗组总有效率为93.18%,对照组为75.00%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组PASI、DLQI评分均较治疗前降低,且治疗组PASI、DLQI评分均低于对照组(P<0.05)。治疗后,2组皮疹颜色、皮疹灼热感、瘙痒、心烦易怒等证候评分均较治疗前降低(P<0.05),且治疗组上述各项证候评分均低于对照组(P<0.05)。治疗后,2组血浆黏度及高、低切血液黏度水平均较治疗前下降(P<0.05),且治疗组上述3项指标均低于对照组(P<0.05)。结论:在西医常规治疗基础上,凉血化斑汤加味能进一步减轻寻常型银屑病血热内蕴证患者的症状体征,改善生活质量和血液流变学状态,提高治疗效果。Objective:To observe the clinical effect of modified Liangxue Huaban Decoction on psoriasis vulgaris with internal retention of blood heat syndrome.Methods:A total of 88 cases of patients with psoriasis vulgaris were collected and divided into the control group and the treatment group according to the random number table method,with 44 cases in each group.The control group was treated with Avi A capsules;the treatment group was treated with modified Liangxue Huaban Decoction based on the treatment of the control group.The changes in Psoriasis Area and Severity Index(PASI),Dermatology Life Quality Index(DLQI),symptom scores of internal retention of blood heat,and hemorheology indexes before and after treatment as well as the clinical effects were compared the two groups.Results:After treatment,the total effective rate was 93.18% in the treatment group and 75.00% in the control group,the difference between the two groups being significant(P<0.05).After treatment,the scores of PASI and DLQI in the two groups were decreased when compared with those before treatment(P<0.05),and the scores of PASI and DLQI in the treatment group were lower than those in the control group(P<0.05).After treatment,the syndrome scores like skin rash color,skin rash burning sensation,itching,restlessness and irritability in the two groups were decreased when compared with those before treatment(P<0.05),and the syndrome scores of internal retention of blood heat syndrome in the treatment group were lower than those in the control group(P<0.05).After treatment,the plasma viscosity and high and low shear blood viscosity in the two groups were decreased when compared with those before treatment(P<0.05),and the above three indexes in the treatment group were lower than those in the control group(P<0.05).Conclusion:Based on the routine treatment of western medicine,modified Liangxue Huaban Decoction can further reduce the symptoms and signs of patients with psoriasis vulgaris with internal retention of blood heat syndrome,improve the quality of l
