术前血清HDL-C、IGF-1水平对前列腺癌根治术患者预后的预测价值研究

摘　　要：目的探讨术前血清高密度脂蛋白胆固醇(HDL-C)、胰岛素样生长因子-1(IGF-1)对前列腺癌根治术患者预后的预测价值。方法回顾性选取2017年8月至2021年10月安徽医科大学附属阜阳医院治疗的90例前列腺癌患者的临床资料,分析患者术前血清HDL-C、IGF-1水平。术后对患者随访1年,评估患者预后情况,分析术前血清HDL-C、IGF-1水平对前列腺癌根治术患者预后的预测价值。结果90例前列腺癌根治术患者中预后不良20例,占比为22.22%。不同预后患者血清IGF-1、HDL-C、总胆固醇、空腹血糖水平比较,差异具有统计学意义(P<0.05)。经Logistic回归分析显示,血清HDL-C、IGF-1是前列腺癌根治术患者预后的影响因素(P<0.05);受试者工作特征(ROC)曲线显示,血清HDL-C、IGF-1单独及联合对前列腺癌根治术患者预后均有一定的预测价值,且联合预测价值更高。结论血清HDL-C、IGF-1可对前列腺癌根治术患者预后产生影响,且术前血清HDL-C与IGF-1联合检测对前列腺癌根治术患者预后具有较高的预测价值。Objective To explore the predictive value of preoperative serum high density lipoprotein cholesterol(HDL-C)and insulin-like growth factor-1(IGF-1)on the prognosis of patients undergoing radical prostatectomy.Methods The clinical data of 90 patients with prostate cancer treated in Fuyang Hospital of Anhui Medical University from August 2017 to October 2021 were retrospectively analyzed,and the preoperative serum levels of HDL-C and IGF-1 were analyzed.The patients were followed up for one year after operation and prognosis condition were evaluated.The predictive value of preoperative serum HDL-C and IGF-1 levels on the prognosis of patients undergoing radical prostatectomy were analyzed.Results Of the 90 patients with radical prostatectomy,20 patients had poor prognosis,accounting for 22.22%.There were significant differences in serum IGF-1,HDL-C,total cholesterol,and fasting blood glucose levels in patients with different prognosis(P<0.05).Logistic regression analysis showed that serum HDL-C and IGF-1 were prognostic factors in patients undergoing radical prostatectomy(P<0.05).The receiver operating characteristic(ROC)curve showed that serum HDL-C and IGF-1 alone and in combination had certain predictive value for the prognosis of patients undergoing radical prostatectomy,and the combined predictive value was higher.Conclusions Serum HDL-C and IGF-1 can affect the prognosis of patients with radical prostatectomy,and the combined detection of serum HDL-C and IGF-1 before operation has high predictive value for the prognosis of patients with radical prostatectomy.
