ERAS理念下的全程导向性管理在经尿道前列腺等离子体双极电切术患者围手术期的应用效果观察

摘　　要：目的探讨快速康复外科(ERAS)理念在经尿道前列腺等离子体双极电切术(TUPKP)患者围术期护理中的应用效果。方法选取2021年7月至2022年12月在重庆两江新区人民医院接受TUPKP的204例患者作为研究对象,采用随机数字表法分为ERAS导向组(n=102)和常规组(n=102),常规组实施常规围术期护理,ERAS导向组实施ERAS理念下的全程导向性管理。比较两组患者术后恢复情况、干预前后患者心理状态[采用心理痛苦温度计(DT)、抑郁症筛查量表(PHQ-9)、广泛性焦虑量表(GAD-7)评估]及舒适度,比较两组并发症情况及护理满意度。结果ERAS导向组首次下床活动时间、首次排气时间、导尿管留置时间、住院时间均显著短于常规组(P<0.05);干预后,两组DT、GAD-7、PHQ-9评分均显著降低(P<0.05),且ERAS导向组低于常规组(P<0.05);干预后,两组舒适度各维度评分均显著升高(P<0.05),且ERAS导向组高于常规组(P<0.05);ERAS导向组术后并发症总发生率显著低于常规组(P<0.05);ERAS导向组对护理满意度显著高于常规组(P<0.05)。结论将ERAS理念下的全程导向性管理应用于TUPKP患者,可促进其术后恢复,改善其不良心理状态,有效提高舒适度和护理满意度评分,降低并发症风险。Objective To explore the application effect of enhanced recovery after surgery(ERAS)concept in the perioperative nursing of patients undergoing transurethral bipolar plasma kinetic prostatectomy(TUPKP).Methods A total of 204 patients who received TUPKP in People′s Hospital of Chongqing Liangjiang New Area from July 2021 to December 2022 were selected as the study subjects,and they were divided into ERAS-guided group(n=102)and conventional group(n=102)by the random number table method.The conventional group received conventional perioperative nursing,and the ERAS-guided group was given whole-course guided management under ERAS concept.The postoperative recovery status,psychological state[distress thermometer(DT),patient health questionnaire-9(PHQ-9),generalized anxiety disorder-7(GAD-7)]and comfort level before and after intervention,and complications and nursing satisfaction were compared between the two groups.Results The first ambulation time,first exhaust time,urinary catheter indwelling time and hospital stay in ERAS-guided group were significantly shorter than those in conventional group(P<0.05).After intervention,the scores of DT,GAD-7 and PHQ-9 in both groups were significantly decreased(P<0.05),and the scores in ERAS-guided group were lower than those in conventional group(P<0.05).After intervention,the scores of each dimension of comfort scale were significantly increased in both groups(P<0.05),and the scores were higher in ERAS-guided group than those in conventional group(P<0.05).The total incidence rate of postoperative complications in ERAS-guided group was significantly lower compared with that in conventional group(P<0.05).The nursing satisfaction in ERAS-guided group was significantly higher compared to conventional group(P<0.05).Conclusions Whole-course guided management under ERAS concept can promote the postoperative recovery,improve the adverse psychological state,effectively enhance the comfort level and nursing satisfaction score,and reduce the risk of complications in patients undergoing
