扶阳保肾方对糖尿病肾病合并Ⅳ型心肾综合征患者血清指标的影响

摘　　要：目的:研究扶阳保肾方对糖尿病肾病合并Ⅳ型心肾综合征患者血清指标的影响。方法:选取120例糖尿病肾病合并Ⅳ型心肾综合征心肾阳虚证患者进行试验,按随机数字表法分为观察组与对照组,每组60例。两组均进行运动、戒烟、吸氧等常规治疗。对照组进行常规西药治疗,包括口服缬沙坦胶囊、胰岛素制剂;观察组在对照组基础上使用扶阳保肾方进行治疗。观察两组治疗效果,记录两组治疗前后中医证候积分,测定两组治疗前后超敏C反应蛋白(Hs-CRP)、肌酐(CREA)、尿素氮(BUN)、血清β_(2)微球蛋白(β_(2)-MG)、脑钠肽(BNP)水平,观察两组不良反应发生率。结果:观察组治疗效果优于对照组。治疗后两组患者各项中医证候积分下降,且观察组心悸、胸闷气短、下肢浮肿、身寒肢冷评分低于对照组,差异有统计学意义(均P<0.05)。治疗后观察组BUN、CREA、β_(2)-MG、Hs-CRP、BNP低于对照组,差异有统计学意义(均P<0.05)。两组不良反应比较,差异无统计学意义(P>0.05)。结论:扶阳保肾方治疗糖尿病肾病合并Ⅳ型心肾综合征效果较好,对肾功能改善明显,且能降低患者血清BUN、CREA水平及炎症反应,未增加患者不良反应。Objective:To study effect of Fuyang Baoshen recipe on serum indexes of diabetes nephropathy patients with type ivcardiorenal syndrome.Methods:120 cases of diabetic nephropathy complicated with type ivcardiorenal syndrome with heart-kidney yang deficiency were selected foRthe experiment,and were randomly divided into observation group and control group,with 60 cases in each group.Both groups received routine treatment such as exercise,smoking cessation,and oxygen inhalation.The control group received routine western medicine treatment,including oral administration of valsartan capsules and the use of insulin preparations to lowerblood sugar.The observation group were treated with Fuyang Baoshen formula on the basis of the control group.Observe treatment effects of two groups and record the TCM syndrome scores before and aftertreatment.Measure the levels of hypersensitive C-reactive protein(Hs-CRP),creatinine(CREA),urea nitrogen(BUN),and serum in two groups before and aftertreatmentβ_(2) microglobulin(β_(2)-MG)and brain natriuretic peptide(BNP)levels.Observe the incidence of adverse reactions in both groups.Results:The therapeutic effect of the observation group were significantly betterthan that of the control group.AfteRtreatment,the scores of TCM syndromes in the two groups decreased significantly,and the scores of palpitation,chest tightness and shortness of breath,edema of lowerlimbs and cold body and limbs in the observation group were lowerthan those in the control group(P<0.05).AfteRtreatment,BUN,CREA,β_(2)-MG,Hs-CRP,BNP in the observation group were lowerthan those in the control group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion:Fuyang Baoshen recipe has significant effect in treating diabetic nephropathy with typeⅣcardiorenal syndrome,obviously improving renal function,reducing levels of serum BUN and CREA,reducing the inflammatory reaction of patients,and not increasing the risk of adverse reactions.
