天竹定眩汤联合“治神调髓”针刺法治疗卒中后眩晕疗效研究

摘　　要：目的:探讨天竹定眩汤联合“治神调髓”针刺法治疗卒中后眩晕的临床疗效。方法:将收治的84例卒中后眩晕患者进行前瞻性研究,按随机数字表法分为观察组、对照组,各42例。对照组给予“治神调髓”针刺法治疗,观察组在对照组基础上给予天竹定眩汤加减治疗。观察两组治疗前后临床疗效、中医证候评分,通过彩超检测脑动脉平均血流速度、相关量表评分、血清神经功能指标、血液流变学指标及安全性评价。结果:治疗后,观察组总有效率为90.48%,高于对照组71.43%,差异有统计学意义(P<0.05)。治疗后,两组中医证候评分、血清胶质纤维酸性蛋白、血液流变学指标、眩晕障碍量表评分均较治疗前降低,且观察组低于对照组,差异有统计学意义(均P<0.05)。治疗后,两组大脑动脉平均血流速度、血清脑源性神经营养因子、血清神经生长因子水平、Berg平衡表评分均较治疗前升高,且观察组高于对照组,差异有统计学意义(均P<0.05)。结论:天竹定眩汤联合“治神调髓”针刺法治疗卒中后眩晕临床疗效较好。Objective:To investigate clinical effect of Tianzhu Dingxuan decoction combined with Zhishen Tiaosui acupuncture on patients with vertigo after stroke.Methods:84 patients with vertigo after stroke were prospectively studied,divided into observation group and control group by random number table method,42 cases in each group.The control group were treated with Zhishen Tiaosui acupuncture,and the observation group were treated with Tianzhu Dingxuan decoction on the basis of the control group.To observe clinical efficacy,TCM syndrome score,average cerebral artery blood flow velocity,scores of related scales,serum neurological function indexes,hemorheology indexes and safety evaluation.Results:AfteRtreatment,the observation group total effective rate was 90.48%higher than that in the control group 71.43%,difference statistically significant(P<0.05).AfteRtreatment,TCM syndrome score,serum glial fibrillary acidic protein,whole blood low tangential viscosity,whole blood high tangential viscosity,plasma viscosity level and vertigo disorder scale score in the two groups were lower than before treatment and the above indicators in the observertion group were lowerthan those in the control group,difference statistically significant(all P<0.05).AfteRtreatment,average cerebral artery blood flow velocity,serum brain-derived neurotrophic factor,serum nerve growth factoRand Berg balance score in the two groups were higher than before and the above indicators in the observertion group were lowerthan those in the control group,difference statistically significant(all P<0.05).Conclusion:Tianzhu Dingxuan decoction combined with Zhishen Tiaosui acupuncture has significant clinical effect on patients with vertigo after stroke.
