摘　　要：目的探讨动态对比增强磁共振血管造影(magnetic resonance angiography,MRA)联合超声微泡造影在下肢动脉硬化闭塞症中的诊断价值。方法方便选取2021年6月—2022年6月齐齐哈尔医学院附属第二医院收治的72例疑似下肢动脉硬化闭塞症患者设为研究对象,以临床综合诊断结果为金标准,明确动态对比增强MRA、超声微泡造影以及MRA联合超声微泡造影在下肢动脉硬化闭塞症发生心脑血管疾病中的诊断价值。结果以临床综合诊断为金标准,72例下肢动脉硬化闭塞症患者中,MRA联合超声微泡造影诊断灵敏度、特异度高于动态对比增强MRA,差异有统计学意义(χ^(2)=4.336、4.247,P均<0.05);MRA联合超声微泡造影诊断灵敏度、特异度高于超声微泡造影,差异有统计学意义(χ^(2)=5.263、6.844,P均<0.05);动态对比增强MRA诊断灵敏度、特异度与超声微泡造影比较,差异无统计学意义(χ^(2)=0.051、0.419,P均>0.05)。下肢动脉硬化闭塞症发生心脑血管疾病中动态对比增强MRA(AUC=0.793)、超声微泡造影(AUC=0.823)、MRA联合超声微泡造影(AUC=0.898)对下肢动脉硬化闭塞症发生心脑血管疾病有预测价值(P均<0.05)。结论动态对比增强MRA联合超声微泡造影诊断下肢动脉硬化闭塞症的符合率较高,对早期明确下肢动脉硬化闭塞症患者病情及发病部位具有积极意义。Objective To evaluate the diagnostic value of dynamic contrast-enhanced magnetic resonance angiography(MRA)combined with ultrasonic microbubble angiography in arteriosclerosis obliterans of lower limbs.Methods A total of 72 patients with suspected lower extremity arteriosclerosis obliterans admitted to the Second Affiliated Hospital of Qiqihar Medical College from June 2021 to June 2022 were conveniently selected as the study objects.Taking the clinical comprehensive diagnosis results as the gold standard,the diagnostic value of dynamic contrast enhanced MRA,ultrasonic microbubble contrast and MRA combined with ultrasonic microbubble contrast in the cardiovascular and cerebrovascular diseases of lower extremity arteriosclerosis obliterans was determined.Results In 72 patients with lower extremity arteriosclerosis occlusive disease,the diagnostic sensitivity and specificity of MRA combined with ultrasound were higher than those of dynamic contrast enhanced MRA,and the differences were statistically significant(χ^(2)=4.336,4.247,both P<0.05).The diagnostic sensitivity and specificity of MRA combined with ultrasound microbubble contrast were higher than those of ultrasound microbubble contrast,and the differences were statistically significant(χ^(2)=5.263,6.844,both P<0.05).There was no statistically significant difference in diagnostic sensitivity and specificity between dynamic contrast enhanced MRA and ultrasound microbubble contrast(χ^(2)=0.051,0.419,both P>0.05).The predictive value of dynamic contrast-enhanced MRA(AUC=0.793),ultrasonic microbubble angiography(AUC=0.823)and MRA combined with ultrasonic microbubble angiography(AUC=0.898)in the incidence of lower extremity arteriosclerosis obliterans in cardiovascular and cerebrovascular diseases(all P<0.05).Conclusion The coincidence rate of dynamic contrast enhanced MRA combined with ultrasonic microbubble angiography in the diagnosis of lower extremicy arteriosclerosis obliterans is high,which has positive significance for the early identification of the di
