摘　　要：目的 探讨胎儿主肺动脉多普勒参数与新生儿呼吸窘迫综合征(NRDS)的关系及预测价值。方法 研究共纳入136例病例,临产前获取胎儿主肺动脉多普勒参数。根据新生儿结局分为NRDS组59例及非NRDS组77例。比较两组胎儿主肺动脉多普勒参数,绘制受试者工作特征曲线(ROC)并计算曲线下面积(AUC),根据最大约登指数获得最大临界值,并计算AT/ET比值在预测NRDS的敏感度、特异度、准确度。结果 NRDS组胎儿主肺动脉PSV、AT、AT/ET比值均明显低于对照组,差异具有统计学意义(P<0.05)。胎儿主肺动脉AT/ET比值在预测新生儿NRDS上的AUC为0.957,根据最大约登指数获得最大临界值为0.833,敏感度、特异度、准确度分别为93.5%、89.8%、92.7%。结论 胎儿主肺动脉AT/ET比值对新生儿NEDS的预测有显著价值。Objective To explore the relationship between fetal aortopulmonary Doppler parameters and neonatal respiratory distress syndrome(NRDS) and their predictive value.Methods A total of 136 cases were included in the study in which fetal aortopulmonary Doppler parameters were obtained prior to delivery.According to neonatal outcomes, 59 patients were divided into NRDS group and 77 cases in non-NRDS group.Comparing the Doppler parameters of the two groups of fetal aortopulmonary artery, the Receiver operator characteristic curve(ROC)analysis was plotted and the area under the curve(AUC) was calculated, the maximum cut-off value was obtained according to the maximum Youden's index, and the sensitivity, specificity and accuracy of the AT/ET ratio in predicting NRDS were calculated.Results The PSV,AT,AT/ET ratio parameters of fetal aortopulmonary Doppler ultrasound in the NRDS group were significantly lower than those in the control group, and the difference was statistically significant(P<0.05).The fetal aortopulmonary AT/ET ratio had an AUC of 0.957 in predicting neonatal NRDS,and the maximum cut-off value was 0.833 based on the maximum Youden's index.The sensitivity, specificity and accuracy were 93.5%,89.8% and 92.7%,respectively.Conclusion The measurement of AT/ET ratio by prenatal aortopulmonary artery Doppler has significant value in the prediction of neonatal respiratory distress syndrome.
