摘  要:目的探讨高压氧联合倍他司汀治疗良性阵发性位置性眩晕(BPPV)的临床疗效及其作用机制。方法选取2021年6月至2022年12月长治医学院附属和平医院神经内科收治的BPPV患者116例作为研究对象, 按照随机数字表法分为观察组和对照组, 每组58例。对照组均予以常规对症治疗及口服倍他司汀治疗, 观察组在对照组治疗的基础上联合高压氧治疗。比较2组患者治疗后临床疗效及不良反应发生情况;分别于治疗前后采用电化学发光法检测2组患者血清雌二醇(E2)水平, 采用酶联免疫吸附法检测血清耳石蛋白-1(otolin-1)水平。结果观察组治疗后临床总有效率(91.38%)明显高于对照组(77.59%), 差异有统计学意义(χ^(2)=4.209, P=0.040);观察组治疗后血清E2水平明显高于对照组, 血清otolin-1水平明显低于对照组(P<0.05或P<0.01);观察组患者治疗后躯体、功能、情感评分明显低于对照组(P<0.01);观察组治疗后不良反应总发生率(8.62%)明显低于对照组(27.59%), 差异有统计学意义(χ^(2)=5.098, P=0.024)。结论高压氧联合倍他司汀治疗可有效改善BPPV患者的临床症状, 提高患者生活质量, 其机制可能是通过调节血清E2、otolin-1水平, 减轻内耳损伤实现的。Objective To explore the clinical efficacy and mechanism of action of hyperbaric oxygen combined with Betahistine in the treatment of benign paroxysmal positional vertigo(BPPV).Methods A total of 116 BPPV patients admitted to Department of Neurology of Heping Hospital Affiliated to Changzhi Medical College from June 2021 to December 2022 were selected as the research subjects and assigned into observation group and control group according to the random number table method,with 58 cases in each group.The control group was treated with conventional symptomatic treatment combined with Betahistine,and the observation group was treated with hyperbaric oxygen on the basis of the treatment the control group.The scores of clinical symptoms and incidence of adverse reactions were compared between the two groups after treatment.The serum estradiol E2 levels before and after treatment were detected by electrochemiluminescence method,and serum otolin-1 levels before and after treatment were detected by enzyme-linked immunosorbent assay.Results After treatment,the total clinical effective rate of the observation group(91.38%)was significantly higher than that of the control group(77.59%),and the difference was statistically significant(χ^(2)=4.209,P=0.040).After treatment,the serum E2 level in the observation group was significantly higher than that in the control group,and the serum otolin-1 level was significantly lower than that in the control group(P<0.05 or P<0.01).The physical,functional,and emotional scores of the observation group were significantly lower than those of the control group after treatment(P<0.01).The total incidence of adverse reactions in the observation group(8.62%)was significantly lower than that in control group(27.59%),and the difference was statistically significant(χ^(2)=5.098,P=0.024).Conclusion Hyperbaric oxygen combined with Betahistine can effectively improve the clinical symptoms and the quality of life of the patients with BPPV,and the mechanism of action may be realized through alleviati
