化瘀消癓汤联合甲氨蝶呤不同给药次数对异位妊娠的疗效及保留输卵管功能的影响

摘　　要：目的 探讨化瘀消癓汤联合甲氨蝶呤不同给药次数对异位妊娠(EP)的疗效及保留输卵管功能的影响。方法 选取2019年9月至2022年9月于辽宁省健康产业集团阜新矿总医院诊治的78例EP患者作为研究对象,按照随机数字表法分为观察组与对照组,各39例。所有患者均给予化瘀消癓汤治疗,对照组采用甲氨蝶呤单次给药治疗,观察组采用甲氨蝶呤多次给药治疗,比较两组的治疗效果。结果 观察组治疗有效率为100.0%,高于对照组的89.7%(P<0.05)。观察组腹痛消失时间、阴道出血停止时间、β-人绒毛膜促性腺激素(β-HCG)恢复正常时间与包块消失时间均短于对照组(P<0.05)。观察组和对照组不良反应发生率比较,差异无统计学意义(P>0.05)。治疗后两组血清β-HCG和孕酮水平均低于治疗前(P<0.05),且观察组血清β-HCG和孕酮水平均低于对照组(P<0.05)。治疗后两组血清抑制素A、白细胞介素-8(IL-8)水平均低于治疗前(P<0.05),且观察组血清抑制素A、IL-8水平均低于对照组(P<0.05)。结论 化瘀消癓汤联合甲氨蝶呤多次给药能提高EP的治疗效果,促进患者康复,且不会增加不良反应的发生,其还可调节患者性激素水平,抑制血清抑制素A、IL-8的表达。Objective To explore and analysis the effect of Huayu Xiaoqing Decoction combined with methotrexate on ectopic pregnancy(EP)and its effect on preserving oviduct function.Methods A total of 78 EP patients diagnosed and treated in Fuxin Mine General Hospital of Liaoning Health Industry Group from September 2019 to September 2022 were selected as the study objects,and were divided into observation group and control group according to random number table method,with 39 cases in each group.All patients were treated with Huayu Xiaoqing Decoction.The control group was treated with single administration of methotrexate,the observation group was treated with multiple administration of methotrexate.The therapeutic effects of the two groups were compared.Results The effective rate of the observation group was 100.0%,which was higher than 89.7%of the control group(P<0.05).The disappearance time of abdominal pain,the cessation time of vaginal bleeding,the recovery time ofβ-HCG and the disappearance time of mass in the observation group were shorter than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the observation group and the control group(P>0.05).After treatment,the serumβ-HCG and progesterone levels in both groups were lower than before treatment(P<0.05),and the serumβ-HCG and progesterone levels in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of serum statin A and interleukin-8(IL-8)in both groups were lower than before treatment(P<0.05),and the levels of serum statin A and IL-8 in the observation group were lower than those in the control group(P<0.05).Conclusion Multiple administration of Huayu Xiaoqing Decoction combined with methotrexate can improve the therapeutic effect of EP and promote the recovery of patients without increasing the occurrence of adverse reactions. It can also regulate the level of sex hormones and inhibit the expression of serum statin A and IL-8.
